Non-Hazardous School Day Trips Agreement

Purpose of trip: Cinema / Theatre / Museum / Coach Trip / City Walk / Shopping / Galleries / Walks

Means of Transport: School Car / Car / Coach / Train / Walk / Taxi / Hired vehicle

Pupil Agreement

I understand and accept that the School’s rules and behaviour policy apply on school trips just as they
do in school and I will be sure to:
• listen to and obey instructions from members of staff or other adult helpers.
• behave in a courteous and respectful manner.
• come correctly dressed as instructed by school staff.

I will not:
• behave in such a way as to endanger others or myself.
• behave in any way contrary to the School’s Rules and Code of Conduct.

I understand that if the Head has reason to think I will not observe the School’s rules, I will not be
permitted to participate and that no refund may be possible.

Parent/Guardian Agreement

Transport – I consent to the pupil travelling by any form of public transport and/or in a motor vehicle
driven by a responsible adult who is duly licensed and insured to drive a vehicle of that type.

Accident/Illness – I consent to all such emergency or other medical or dental treatment including
inoculations, general or local anaesthetic, surgery or blood transfusions which in the opinion of a
qualified medical practitioner are necessary for the safety and wellbeing of the pupil. I agree to the
duty nurse/supervising member of staff administering appropriate first aid or medical treatment whilst
participating in school activities.

Medical Information – My daughter/ward’s medical information and consent is registered and current
with RMS Health Centre. I understand that it is incumbent on me to inform the Health Centre of any
changes to information previously provided by me. If your daughter/ward will be self-administering any
medication sent from home, e.g. travel sickness tablets or paracetamol, the trip leader must be made
aware.

Activities – I agree to the pupil taking part in any/all activities described in the information I receive
about a trip and which fits within the parameters of the purposes of the trip as shown above.
(A separate consent form will be required for residential, overseas and any physical (hazardous)
activities.)

Personal Effects of the Pupil – I acknowledge that the pupil will be responsible for the safety of their
money and personal effects and I will not hold the School responsible for losses unless caused by the
negligence of the School.
Payments made by the School – I understand that in very rare circumstances the School may have to
pay “on the spot” fines or compensation for damage caused by the pupil or other sums of money
necessarily incurred for the health or general welfare of the pupil. I will, within 30 days of request,
refund any such sums to the School and I understand that the School will assist in pursuing any
insurance claim that may be available.

I confirm that I have read all the information provided by the School and the records provided by me
to the School are correct.

I confirm that my daughter/ward has read and understood this document.

I agree to contact the School Health Centre should there be any changes to the medical information
previously provided by me and held by the School in the Health Centre.
