Brief Description:
Patient was in her usual state of health 2 days back when he suddenly developed right sided
weakness involving both upper and lower limbs, without any associated symptoms of facial

and now she can't move her right arm or leg at all. Patient is known diabetic and hypertensive for
last 7 years but thee is history of poor control of blood sugar and BP due to poor compliance. CT
brain shows right parietal infarct. Carotid doppler and EChocardiography planned, medicines
started and counselling of the patient and family done.

Brief Description:
Patient is a known c/o Dementia of Alzheimer's type for the last 3 years having symptoms of
memory disturbance, gradual decline in higher mental and executive functions, irritability, social
avoidance and decreased interest in family matters and daily activities. His relatives came for a
worsening in behavioral disturbance for the last 1 month as he shouted and showed unusual
suspiciousness towards family members. No other psychotic symptoms reported as such.
Baseline labs showed no new physical derangement.

Brief Description:

Brief Description:
Patient was in his usual state of health 2 weeks back when he had symptoms of upper respiratory
tract infection that continued as patient only took symptomatic treatment. Later 3 days back
patient started having symptoms of episodic state of confusion and irritability, also c/o increased
muscle stiffness and pains with extreme weakness and malaise. The symptoms of confusion
worsened and patient became drowsy for 1 day with neck rigidity and fever. CSF showed

changes.

Brief Description:

Brief Description:

Brief Description:
Patient had a history of having attacks of jerky movements of all the limbs since early childhood.

Brief Description:

Brief Description:
Patient was diagnosed with type 1 diabetes since the age of 14 years was taken to the emergency
room because of drowsiness, fever, cough, diffuse abdominal pain, and vomiting. Fever and
cough started 2 days ago and the patient could not eat or drink water. On examination he was
tachypneic, his temperature was 39° C (102.2° F), pulse rate 104 beats per minute, respiratory
rate 24 breaths per minute, supine blood pressure 100/70 mmHg; he also had dry mucous
membranes, poor skin turgor, and rales in the right lower chest. He was slightly confused.His
labs showed white blood cell count 18,000/ μ l, glucose 450 mg/dl. The result of the strip for
ketone bodies in urine was strongly positive. Urinalysis showed glucose 800 mg/dl.

Brief Description:
Patient was diagnosed with COPD 5 years ago. He presented with worsening dyspnoea, cough
and increasing purulent sputum production over the past 3 days. On examination, BP is 130/84
mmHg, pulse 102, respiratory rate 18 and temperature 37.8 ºC. Auscultation of the chest
revealed widespread expiratory wheeze and inspiratory coarse crackles in the left lung base. He
is dyspnoeic but able to speak in whole sentences. No cyanosis is present. He is a known chronic

Brief Description:

Brief Description:
