to HSE, We can realize that the main causes of incidents are due to
bad procedures, this due to misinformation about what is being
done, omission of steps or simple carelessness.
For all these reasons, it is important to carry out a risk
analysis, in order to have control over what may happen
during the operation. For example, in drilling, it is
necessary to have preoperational meetings every day
until the end of the project in order to disseminate
the program to follow and clarify doubts about it, thus
avoiding staff misinformation.
