I, PERCIVAL PORLUCAS, of legal age, Filipino, married, and a resident of

Bawa, Gerona, Tarlac, after having duly sworn to in accordance with law do hereby
depose and state and undertake that:

1. I am the Municipal Government Department Head I (LDRRMO) of the

Municipality of Gerona, as such, all properties in the said Department are now
under my custody and responsibility;

2. That one of the properties to which I am accountable is an ambulance

I, FRANCISCO MANUEL MADRIAGA, of legal age, Filipino, married, and a

resident of Brgy. , Gerona, Tarlac, after having duly sworn to in accordance with law
do hereby depose and state and undertake that:

1. That I am the accused in NPS Docket No. III-18-INQ-18J-0833 for Physical

Injuries, filed before the Office of the Provincial Prosecutor, Tarlac City;

2. That I undertake to appear before the Honorable Court whenever so required by

the Honorable Court or required by the rules;

3. That I have no intention whatsoever to Jump Bail and I undertake to attend all
hearings of the case;

4. That in the event that I have failed to attend any hearing of the said case after
due notice and without any valid justification the same shall be considered as
waiver on my part to be present during the said hearing and the hearing maybe
proceed without my presence;

5. That I am executing this affidavit in order to established the fact that the truth
of the foregoing and for all legal intents and purposes it may serve.;
