NURSING COMMUNICATION

IN HEALTH TEACHING

CASE:
Dindra (15 years old) was treated in a children's unit. He
suffered from gastritis. Ifah / Wanda is a nursing student
who practices in the pediatric ward, will provide health
education to Mrs. Ifah / Wanda (Miss. Dindra).
Pre Interaction phase:
- collect the client data
Read the client’s status (name,age,sex,etc.)
- explore your feeling, fear, and competency
- make a plan to interact with client

Orientation phase:

Greet and smile to the client


Nurse : good morning/afternoon Mrs Ifah/Wanda
Client : good morning/afternoon, nurse

Introduce your self


Nurse : I would like to introduce my self, My name is
Ifah/Wanda., I am a nursing student at UGM
Ask the client would be like to be called
Nurse : What would you like to be called ?
Client : Mrs. Ifah/Wanda , please
Explain the responsibility of the nurse and the client
Nurse : Ok mrs. Ifah/ Wanda, I practice here from
07:00 o’clock to 14.00 o’clock to take care of
your daughter,I hope you could cooperate so
that your daughter could be health again as
before.
Client : okey nurse
Explain the role of the nurse and the client
Nurse :In here, I work with other health profession
such as physician, physiotherapist, dietitian.
You may ask anything about treatment,
particulary about something that you don’t
understand
Client :thank you ners,
Explain the activity that will be conducted: health teaching
Nurse :I will discuss about your daughter who has got
a gastritis
Explain the purpose of health teaching
Nurse :it is important to do so that after the treatment
in the hospital, you could treat your daughter at
home, so your daughter will not suffer from this
disease again
Explain about the time to do health teaching
Nurse :it will take about 10 minutes
Client :It’s okay
Explain about the confidentiality
Nurse :I will keep the confidentiality of some parts of
this discussion if you fell un comfortable, so
would you please not hesitate to express your
feeling/problem
Client :Thank you nurse
Working phase
Give a chance to your client to ask
Nurse :Is there anything you want to ask or to do before
we start ?
Client : Not,now
Start the health teaching in a good way
Nurse :Could we start right now mrs. Ifah/ Wanda
Client :Yes please
Do the health teaching
a. assess the client’s knowledge about the disease
Nurse : Mrs. Ifah/ Wanda have you ever heard
gastritis ?
What do you know about gastritis?
Do you know causes, sign and how to take care
that?
Client :sorry I don’t know exactly
b. explain the necessary things
Nurse :Gastritis is inflammation, irritation, or erosion
of the stomach lining. Can occur suddenly
(acute) or gradually (chronic).
The causes could be Eating spicy foods,
Extreme stress, or can caused by bacteria and
viruses
The signs is Stomach upset or sick, Nausea and
vomiting, Feeling full or burning in your
stomach, Loss of appetite
The first treatment is it sit in a relaxed position
of 35-45 degress, avoid lying down because
stomach acid will incarease, and than give
drink warm water a little and not to fast
because it can trigger vomiting and if the child
has pain in the stomach you can compress it
with warm water using a towel or compress
bag, it’s take time 10-15 mnt. Give soft foods
like porridge, then give antacid drugs for first
aid.
If your child appears to be dehydrated, has
severe stomach pain, continues to nausea
vomiting, cannot drink fluids, or is not aware of
bringing your daughter to the nearest hospital
immediately
But you can reduce the risk of getting the
disease by:
Have good hygiene habits, especially hand
washing. Don't eat or drink things that can
irritate your stomach lining, these include
alcohol, caffeine,sour foods,and spicy foods. Do
not take drugs such as aspirin and sick
medicines in the long term

Nurse :Mrs Ifah/Wanda could you explain what we
