UNDERTAKING, RELEASE AND QUITCLAIM

KNOW ALL MEN BY THESE PRESENTS:

I, ___________________Filipino, of legal age, with residence address

______________________________ do, by these presents, acknowledge that my contract of
employment with __________________________, (the “Company”) has ceased effective
_________________________, in connection with which I declare the following:

1. I acknowledge that I have received all amounts that are now or in the future may be due me from
the Company, including the amount of PESOS: _____PESOS & 0/100 ONLY (₱0.00), Philippine
Currency, representing my final pay;

2. I hereby unconditionally and absolutely release, remiss, and forever discharge the Company, its
affiliates and subsidiaries, their officers, trustees, directors, agents and employees, including their
assigns, representative and successors in interest, from any and all claims or actions of whatsoever
nature, including but not limited to claims for sum of money, or other obligations arising from any
contract or agreement with the Company;

3. I further agree that this UNDERTAKING, RELEASE AND QUITCLAIM may be pleaded in bar to any
suit or proceeding (Civil, SSS, Phil-Health, Labor, etc.) to which either I, or my heirs and assigns,
may have against my employer in connection with my employment with the Company;

4. I hereby warrant that I will not at any time, in any manner whatsoever, directly or indirectly,
commit any act prejudicial to the interest of the Company, its affiliates and subsidiaries, their
stockholders, officers, directors, agents or employees, nor will I disclose any confidential or
proprietary information concerning the business of the Company and its affiliates and subsidiaries,
their manner of operation, plans or data of any kind, as well as personal or confidential information
pertaining to or about any of the employees, officers and directors of the aforesaid entities;

5. I will immediately transfer possession and convey any and all titles, rights and interest over any all
properties (real or personal), machineries, files and folders, that I may be presently holding in
behalf of or in trust for the Company, to any person or entity designated by the Corporation;

6. In the event that I violate the confidentiality or any of my undertakings, commitments and
representations contained herein, I understand that the Company may bring such legal actions
against me as may be allowed by law, and I agree to pay all liabilities and to indemnify the
Company, its affiliates and subsidiaries, their officers, directors, representatives, employees and
assigns for any loss, damage or expense which they may suffer by reason of such violation;

7. I finally declare that I have read this document and have fully understood its contents. I further
declare that I voluntarily and willingly executed this UNDERTAKING, RELEASE AND QUITCLAIM
with full knowledge of my rights under the law.
