1) Via your campaign, which at this writing has raised almost $93,500:
1) The 12 graphics do not link to the claimed media reports and I cannot locate them. Would you
please provide me with the links? Also, what are the names of the people who provided the three
quotes which were apparently published in the MailOnline, Yahoo!, and Unilad stories?

2) Would you please provide me with the information I requested in my previous e-mails?

Before I jump in, I may have incorrectly addressed you in my previous e-mail. If so, my apologies. Is
your legal name Limor Leifenberg or Limor Kuznits?

1) lifewand is the subject of a pending international patent application (not yet pending in the United
States) which is not yet published

A few minutes ago I noted that the "patent pending" claims in the text and apparently in a related
graphic in your Indiegogo fundraising solicitation http://tinyurl.com/yyv7n67e have been altered
without providing donors (including me) or potential donors an explanation of the changes.

Would you please provide me with a detailed list all alterations made to the funding solicitation since it
was first published, including the dates of the alterations?

2) With what agency in what country was the patent application filed and on what date, please?
3) Per my previous inquiry, your fundraising solicitation features the logos of a number of publications
with accompanying text, for example:

When I saw those logos and quotes (without links to their sources), I (and perhaps other donors)
assumed -- reasonably so in my opinion -- that your anti-choking device had been the subject of news
stories reporting by those publications.

However, based on these links you sent me, my assumption was wrong:

I'm guessing Yaron Leifenberg is your husband, is that correct? What are his medical credentials and
has he any previous experience in the field of medical devices? If so, any details would be welcome.

6) What type of business entity is ExtraLife -- sole proprietorship, corporation, LLC? -- and in what
U.S. state is it registered? And what's the name of the registered agent, please?

7) Via the ongoing Indiegogo crowdfunding campaign http://tinyurl.com/yyv7n67e

Is Dr. Glassenberg the author of any research studies re: foreign body airway obstruction? If so, would
you please send me citations or better yet, copies of the studies?

8) My understanding is that video game entrepreneur Sam Glassenberg of Evanston, IL, is Dr.
Glassenberg's husband. Is that correct?

If so, do you, your husband, or other Lifewand/ExtraLife executives have any professional connections
to him? If so, would you please provide details?

Also, does Mr. Glassenberg have any professional/financial connection to the Lifewand?
According to the FDA https://www.accessdata.fda.gov/scripts/cdrh/cfdocs/cfRL/rl.cfm?
lid=598207&lpcd=GCX your device is classified as a Category 2 device.

And also https://www.accessdata.fda.gov/scripts/cdrh/cfdocs/cfCFR/CFRsearch.cfm?FR=880.6740

As I understand it, for this category, FDA registration does not require any oversight of a device's
manufacturing process or any examination of any testing.

Therefore, I'm puzzled by this from the Indiegogo fundraising campaign:

9) Who is responsible for the above claim, please?

10) In my opinion, many if not most potential donors would interpret the above claim to mean that
information about Lifewand's manufacturing process and testing has been provided to the FDA -- at
least that's how I interpreted it. To save me the time/effort filing a FOIA request, would you please
send me copies of all records about the Lifewand that were filed with the FDA?
