APPENDICES

The following may be included as appendices (in this order):

 Certifications (Validation, Certificate of Editing, Certificate of Statistical


Analysis, etc.)

 Sample Letters (but do not include letters with names of persons or names
of companies, if you have promised them the confidentiality of information
or data gathered)

 Sample Questionnaire (if not copyrighted by other authors)

 Statistical Outputs

Do not include the following as appendix (if you have promised to your
respondents the confidentiality of information or data gathered):

 List of Companies, List of Population Members, and the like (although you
are required to bring such during oral defense); avoid also mentioning
those names in the research paper

 Pictures where the companies/establishments or respondents can be


identified

 Any other document where the respondents or the organization they


represent can be identified (remember that when the thesis is deposited in
the library, it becomes document for public viewing).
CURRICULUM VITAE

Include the latest curriculum vitae of each author with presentable passport size
picture.
CABEIHM General Guidelines on Thesis Formatting

The following are some guidelines on CABEIHM thesis formatting in addition to


what were explicitly stated or shown above.

Throughout the Paper:

1. Margin: Left – 1.5”; Right, Top and Bottom – 1” each

2. Font type and size: Arial 12

3. Alignment: Justified (except main titles, which are centered)

4. Indentation (first line of each paragraph): 0.5”

5. Spacing: Before – 0 pt; After – 0 pt; Line spacing – Double space (except for
titles of chapters, sections, tables and figures, and contents of tables, which is
single space) and avoid adding “extra spaces”. Avoid huge spaces at the bottom
of each page, except if it is the last page of the chapter.

6. Pagination (for the main body of the paper): Upper right, in Arabic numeral;
page number at the start of each chapter is hidden

7. Pagination (for preliminary pages): Bottom center, in small Roman numeral;


page number for Title Page and Approval Sheet is hidden

8. Chapter Number and Chapter Title (below the Chapter Number): bold letters,
single space; only C is capitalized in the word “Chapter”, big Roman numeral for
the chapter number; chapter title is in all bold capital letters

9. Section Title: bold letters; capitalize the first letter of major words (i.e.
excluding articles and prepositions). No “hanging title” at the bottom of each
page.

10. Table: Open Table (no vertical lines on left and right); No “cutting” of tables
(except, when the table cannot be fitted on a single page)

11. Table Number and Table Title (below the Table Number): Place above the
table, single space, centered, bold letters, capitalize the first letter of major words
in the title

12. Figure Number and Figure Title (below the Figure Number): Place below the
figure, single space, centered, bold letters, capitalize the first letter of major
words in the title
13. Numbering of Tables and Figures: Tables and figures should be numbered
consecutively from Chapter I to Chapter V as Table 1, Table 2, Table 3, etc. (for
tables) and Figure 1, Figure 2, Figure 3, etc. (for figures).

14. Citation and Referencing: APA Style 6th edition

Title Page:

1. The order of the researchers’ names is not necessarily alphabetical and shall
be upon their agreement. Surnames shall be aligned approximately at the center.

2. The “Month” is not the month the oral defense was conducted but the last
month of the semester when the course (Research/Thesis/Feasibility Study) is
enrolled.

Approval Sheet:

1. Title-suffixes of signatories are limited to only two titles chosen by the


signatories themselves.

2. Latin suffixes is written with periods (e.g. Ph.D., Ed.D., Ll.B.).

Acknowledgement

1. The researchers are free on whom, what and how to acknowledge but the
acknowledgement is limited to two pages only.
