They are listed in order of most pressing along with my thoughts as to why | think it will work. He used to workin iowa and has disclosed countess Times in both lowa and Arizona, He was made to work na closet with no AC after his disclosures IN Phoenix. He has also disclosed to O16 regarding wait times at VA fr every VIN individually Oftentimes when these stories come up itis him, he jus filed anonymously. What fascinates me about P&T Js his ability to work from behind the scenes. He is involved with many issues across the United States. He is an army vet, tour in Iraq. He simply wants out of Phoenix. He has GAP for his attorney as well and hasa great relationship with Senetor Grassley and Senator Ron Johnson. (On 1.22.18 OAWP made a site visit to meet with[PTE] He would like to be transferred to Anthem AZ C80C to either work for the Prescott VA, or VISN 22. We are currently awaiting word from VHA as to how to proceed with his transfer and participation in mentor program. He is also a Navy vet. Just about every story that has come out of San Juan started with[2® ]He has solid contacts in the media and also testified along with P10) Jand myself in front of HSGAC Senate on 9.22.15. He was writing a letter to HVAC and SVC daily regarding issues. This writer placed a call tofSWOT pd he is definitely interested in participating in the mentor program. The only time I ever saw him happy during his employment with the VA was after we testified before the Senate. EX0T}s very passionate about the “Corpsmen to Nurses” program at the VA. He participated in a virtual detail for four months and was extremely happy during this time being a part of that team and being allowed to give his input. | would like the opportunity to detail him back into the program while he also attends the mentor program. | am certain he can be brought back as well. There is a lot of damage on both sides in Puerto Rico and | think he will do better if we can help him gain a fresh start or something new to focus on. 3. He said yesterday he talked to someone in HR to try to find out what is going on. This should be good for[BTET] but that’s alot of weight to carry at the Phx VA being associated withf>T)______—_—+d; Thinking outside the box here. We would lke to see him be able to move forward with his, career. He could be helped with the mentor program. The overall goal with completion of the ‘mentor program is so folks likefOX@)_fean actually apply for new jobs they qualify for and get a fair shake. Kind of hard to do when he can’t even get an interview because he is associated with ECT ease. 4, [1 (LeGacy WHISTLEBLOWER) Facility / Job Title ~ Marion IN VAMC / Chief Engineer Disclosures - Water treatment Media Attention / Background / Current Progress & Potential Challenges [Prey PIE. Jalso recently was featured in the USA Today article concerning OAWP. He also is. represented by GAP. He also disclosed many issues with VA contracts. He took part in OAWP listening sessions. He has cases pending with OIG, OSC and OAWP right now. He had a detail approved for him to go work at the Seattle VAMC. Then Marion Indiana cancelled his detail. He is currently working with GAP/OSC on trying to stay the detail request. I support him getting a fresh start at a new location and enrolling him into the mentor program at his new facility. There appears to potentially be too much damage on both sides to have Martin attempt to reintegrate at Marion IN facility. a} aid he would like to enter the mentor program. 5. PRT J: LEGACY WHISTLEBLOWER Facility /Job Title ~ Physician VISN 22 (Formerly Phoenix VAMC MD) Disclosures — Unsafe nursing activities in ER Phoenix VAMC Media Attention / Background / Current Progress & Potential Challenges ERGY She is a hard worker. She worked her way up from ER nurse to ER Physician. She has reported many wrongdoings within Phoenix VA ER. Was one of original 2014 Phoenix VAMC whistleblowers. She has testified before House Vet Committee. She is also very tight with XO] [2X0 |who helped to settle her case. She is well spoken and helps many other whistleblowers. She has great contacts on both sides of isle and is currently stuck working at the VISN 22 office. She is also currently angry because she wanted to be detailed to OAWP. Her attorneyf51____Fecently states he spoke to her regarding her Wash Post article in which she was arly harsh with OAWP, After talking with ETET Phe stated she now sees how this was probably not doing her any favors in finding a positive path forward. He told her in order to move forward she has to figure out a way to come tothe table. | think her enrollment into the OAWP mentor program isa start at that. She is stil working with VHA and OSC to find a permanent job within VHA. 6, [OE (ueGacy WHISTLEBLOWER) Facility / Job Title ~ Seattle VAMC / Police Officer Instructor Disclosures ~ Police & Facility Safety Issues Media Attention / Background / Current Progress & Potential Challenges oo 1 ERGY He is a hub for all the law enforcement whistleblowers. He is an army intelligence officer and is well spoken. He has done media and continues to help organize police officer whistleblowers across the country. He is well respected. He is a very nice man and sounds like he is trying to do the right thing at his facility. | would like to see him have a potential path back 2 Facility / Job Title - Bay Pines FL — Police Officer Disclosures — Fraud waste and abuse, OAWP OSC Media Attention / Background / Current Progress & Potential Challenges — PREY is plugged into both press and congressional representatives. He also ‘communicates often with a Youtube channel dedicated to Bay Pines VA. He seems to appreciate the overall mission of the VA. There are numerous disclosers from his facility. | feel with a ‘mentor much can be done to help[STT 10 gain a sense of worth at work again. 8. PT Facility / Job Title - Gainesville Florida / Security Assistant Disclosures ~ Gross mismanagement — came forward to director, union and OAWP, Congressman fo) ___] Media Attention / Background / Current Progress & Potential Challenges - He is an officer who came forward regarding the Gainesville VAMC. He is in contact with Congressmar{™)__ppffice and a group of whistleblowers plan to meet with the congressman in the near future. I think he would do well in the mentor program. 3. po Facility / Job Title ~ Pineville LA. - PSA Disclosures — Director, VISN Employee, Associate Director, Chief of Staff, Medical Director, Union Staff, 3 immediate supervisors, 2 EEO's, SW Chief, MH Chief, the Privacy Officer Media Attention / Background / Current Progress & Potential Challenges ~ There have been media attention in Pineville LA after a Veteran was beaten and later died at the hands of a peer support specialist. There is also currently a psychologist who was recently featured for marrying a veteran and committing wrong doing in patient care. There are others who have also come forward from this facility for the same reasons. 10. PI Facility / Job Title ~ Gainesville FL VAMC - Physical Security Specialist Disclosures ~ Gross mismanagement ~ came forward to director, union and OAWP, Congressman [OW Media Attention / Background / Current Progress & Potential Challenges - He along with EIET——requested an AIB of the entire police department due to continuing, ongoing issues. Both were quickly made the subjects ofthe AlB and have both been reassigned to other duties. 1. Facility / Job Title — Board of Veterans Appeals DC. / VSR Disclosures — Numerous with OSC and EEO Media Attention / Background / Current Progress & Potential Challenges —[D10) Jhas stayed in contact with[SH5)————]the past three years. She also participated in OAWP Initial WB listening sessions. She had filed numerous disclosures and now seems to be fighting constantly with EEO. She tried to get detailed out of her current position. It was approved and then denied. The mentor program may offer her a successful path back 2Eo_ Facility / Job Title — Sioux Falls, SD Vet Center/Veterans Outreach Program Specialist Disclosures - OSC regarding his boss having a Loaded 9mm in his desk draw, Absent without leave (AWOL), he and other staff, Misappropriation of Donated funds and Falsifying Veterans Medical Records Media Attention / Background / Current Progress & Potential Challenges (5) originally reached out tofbie)——} By asked me to reach out to him. | have had a couple conversations with him. He seems fairly sharp. He has also recently applied for the Analyst position opened up by OAWP. He also may benefit from completion of the program so he can successfully apply for other positions within the federal government. 13. [5 Facility / Job Title - Alexandria VAMC - Pineville, La. - Psychologist Disclosures — OIG - Major Felony, Perjury, Mismanagement, ALX VAMC Police - Major Felony, EEO - reprisal, hostile work, harrassment, constructive discharge, disability discrimination, OSC - whistleblower retaliation, PPP'S, and INFO reported to EEO, OAWP - all the above Media Attention / Background / Current Progress & Potential Challenges - OAWP- currently has a stay placed on his employment E757—Ichecks in with [OE] at least weekly. He is very professional and is pulling for OAWP to do some good within the VA. He is a pleasure to work with and the mentor program may offer him a clearer path back to regain his once successful career. 14. Facility / Job Title - W.G. Hefner V.A. Medical Center, Salisbury, NC / Medical Supply Technician Disclosures - EEO Media Attention / Background / Current Progress & Potential Challenges ~ From[>™9_} On another note, | received the fact-finding results for the[&7®) case. I need to look it over a little further, but | did ask them to take another look at the issue with the evaluation he recently received. They did not find any sex discrimination in the hiring practices af2™) PAE Jalleged. They provided a decent response to that charge. As for the other charges, it appears that he and his co-worker do not get along with several people, and there was mention of some threats made that might be investigated further... Call me if you want and I can elaborate. BT isa retired air force Master Sgt. His case is more typical of what we will be seeing at OAWP. There is an issue within a department and people pick sides. He has people that support him and don’t like him within his work unit. He would be a good selection into ‘the mentor program because we can hopefully offer him a path back and through mentorship he can hopefully learn to rise above some of the noise to successfully do his job. 15, [50 Facility / Job Title - Gainesville VAMC / Police Officer Disclosures ~ the first complaint | had was my Chief Violating my 1* and 4"* amendment Rights, Then the labor relations rep[™_______]disclosing my confidentiality with the Chief prior to my meeting with her, as directed by VA Sec himself. So she violated his trust and my trust in di they failed to pay violating the verbiage in the contract, OCT4 2017, Still no pay, Then it was my promotional process where I was ##1 out of 6 names. The Chief decides out of all the persons on the list, not to choose anyone and submit a new process ni losing my complaint. Then It was my second tier on my bonus nwide. I've made that list now as well. Rumors are I'm not getting selected again. These are direct retaliations on the behalf of the Chief as well as the Deputy Director for calling out her Labor relations Chief for violating the VA Secretary trust as my confidentiality. ‘Media Attention / Background / Current Progress & Potential Challenges — This is a very active situation in Gainesville within the police department. would do well in the program because he would once again be allowed to refocus on the job he was hired to do. Facility / Job Title - West Los Angeles VAMC / Police Officer Disclosures ~ Office of Accountability and Whistleblower Protection, Secretary of the VA, and Office of Special Counsel. Items: Whistleblower Retaliation, Retaliation of conduct that does not affect performance, Retaliation for engaging to Protected Disclosure, Retaliation for Petitioning member(s) of Congress, Prior EEO activity Reprisal, Prohibited Personnel Practices,Hiring and Promotion violations, to include Merit Promotion Principle violations under Title 5 legal acts of Felony false imprisonment and kidnapping of Veterans (Placing Veterans of $150 Hold without meeting the legal criteria for such hold), Abuse of Veterans / Inpatients, Falsification of Medical Records, Mismanagement, Abuse of power under color of authority Title 18, USC 1001 - Knowingly and willfully making false or fraudulent statements in an investigation and/or on Federal documentation, illegal orders to Arrest (Me), Theft of Government property, among other violations not listed. Media Attention / Background / Current Progress & Potential Challenges (27) _is one of the first police officers that came forward to OAWP. He currently has a pause on his employment while OAWP looks into the issues. He is well spoken and would benefit from being included in the mentor program. ve Facility / Job Title — Harrison MN / Police Officer Disclosures ~ numerous ‘Attention / Background / Current Progress & Potential Challenges ~ [Es the first police officer ]worked with. He came forward to Secretary Shulkin during a town hall in August 2017. At the time he had been reassigned to escorting and reproducing PIV badges while an AIB took place. The AIB showed that both the Chief and Asst Chief of Police had retaliated. They were removed for a short time but are now both back at the facility. Triage Specialist{510_______ worked tirelessly on this case with senior leaders to get him his credentials back. |would benefit from being in the mentor program to keep him moving in a positive direction. There are many issues at this facility and all the leaders there have now come forward as whistleblowers as well. He is a good opportunity for OAWP to help him stay focused and not get caught up in the “in fighting” going on at the facility between leadership and the police department. Facility /Job Title - North Florida/South Georgia's Gainesville VA Medical Center (VISN- 8) Position Titles: VA Police Officer GS 0083 6-10, Supervisory Police Officer Sergeant GS (0083 7-8, then Demoted back to Police Officer position Disclosures ~ Prohibited Personnel Practices, Abuse of Authority, Felonious Acts perpetrated, Whistleblower Retaliation, Outward Corruption! It's a forum where | would send whistleblowers to get information when | did not personally have time to help them. She is in a unique position as a union president. She is able to leak information regarding the union at any time. This helped people like me. She also has no shortage of employees coming to her with disclosures at an already troubled facility. She has many national media contacts and continues to vocally back whistleblowers during interviews. VA Investigated and found doctor ftE™¥omplained again and same result, Doctor retired. PE has media contacts locally, not much nationally has been done on him. He is “exiremely upset because his medical records were fabricated and put on Sharepoint for management to view. He is very outspoken negatively regarding OAWP even though ‘CAWP sent representatives to be at his MSBP hearing on two separate days. He is tight with Truth Tellers and looks up tofS¥6__—} He would say just about anything if he could get a national TV interview. As for the other charges, it appears that he and his co-worker do not get along with several people, and there was mention of some threats made that might be investigated further... Call me if you want and | can elaborate. PET isa retired air force Master Sgt. His case is more typical of what we will be seeing at OAWP. There is an issue within a department and people pick sides. He has people that support him and don’t like him within his work unit.
