Innovative Clinical Fellows - Job Description

Grade: Trust Doctor (specific grade to be confirmed on appointment – FY2 to ST3+ posts available)

Job Summary:

These are new non postgraduate training roles, which strengthen our operational Core and Registrar level Trust
Grade Firms. There are three options for this role.

1. 80% of your time working fully clinically in a main speciality and 20% of your time in an area of special
interest or undertaking a PGCE (as defined in the scheme document)
2. 100% of your time in main speciality – with three months out to work as an Expedition Medic with Raleigh
International (if accepted on to their programme).
3. 100% your time in a main speciality with a flexible contract (to be agreed).

Emergency Medicine is at the heart of the hospital’s emergency care pathway and provides high quality care for
patients from the moment they arrive at the hospital. The department is led by a highly motivated group of
consultants who are committed to providing high level care with a drive to develop the service further. This role will
enable the Emergency Department to deliver high quality and timely care in the face of the pressures resulting from
increased emergency admissions and the greater acuity of cases presenting. The department has been inspected
twice by the CQC in the past 12 months and while the CQC reports for the provision of Emergency Care is described
as Inadequate, this reflects the challenges faced by the Unscheduled Care Pathway rather than solely the
department. The Trust has been supported by the Emergency Care Improvement Programme (ECIP) and through
this the department was inspected by Matthew Cooke, who provided the Trust with a highly favourable report
concerning the ED. In it he highlights the huge positives around the department and the forward thinking approach
to Emergency Medicine delivered by the Emergency Department.

It is a consultant led service with consultants providing leadership and patient care on the shop floor from 08:00 to
00:00 Monday to Friday and 22:00 at weekends with a desire to enhance weekend cover. The ethos of the
department is to provide high level care whilst supporting the education and training of junior staff both clinical and
nursing both on and off the shop floor.

These roles will provide the 24/7 registrar cover a department of this size needs. Although this is a Trust grade job
and not part of a formal postgraduate training programme, one of the key benefits will be to increase senior shop
floor presence, whilst allowing the same time for learning and level of support as it you were on a postgraduate
programme.
Key Responsibilities:

Main Speciality – the exact duties will depend on the speciality firm you are placed with – this list therefore is
indicative – 80% of working time or 100% if taking the break option.

Duties will include

1. Delivery of DCC to patients arriving in the Emergency Department


2. Providing support and teaching for Foundation Doctors working in General Medicine including carrying out and
supervising bedside procedures
3. Ensure discharge summaries are completed in a timely manner and communicate with GPs and other clinical
colleagues as required.
4. Maintain continued professional development.
5. Attend educational and multidisciplinary sessions.
6. Comply with all local policies including dress code, annual and study leave
7. Participate in weekend and evening on call rotas in accordance with firm’s rota.

Although this post is not approved for postgraduate training the duties are the same as the existing specialty
training posts. The successful candidate will be assigned a consultant clinical supervisor and will be encouraged to
maintain an educational portfolio with appropriate assessments to their level of experience. There are regular
departmental lunchtime teaching sessions which are open to all medical staff to attend. There are also a wide range
of opportunities to participate in clinical audit and improvement projects.

Selected Specialty – again the exact nature of the work will depend on your selected speciality and may include
(likely to be 20% of the working time)

1. Undertaking supervised work with a specified group of patients


2. Undertaking a research project and presenting back findings to specified clinical and management groups
3. Creating, leading and implementing an approved medical initiative.

OR

3 months as an Expedition Medic with Raleigh International if you are accepted on to their programme, as part of
their independent separate application and selection process.

Raleigh International is a sustainable development charity, and are involved in improving health and hygiene in
communities as well as empowering a generation of champions of the natural environment. Expedition locations
include Tanzania, Nepal, and Costa Rica / Nicaragua. The doctors are well supported and have a senior expedition
medic on-call at all times.

During this time you would continue to be paid, but the fellow would be expected to pay for flights and all
volunteers costs related to the expedition.

Person Specification
Qualifications

- Full GMC registration with a licence to practice


- Primary medical qualification recognised by the GMC
Clinical Experience

- Evidence of completion of Foundation Level or equivalent 2 year training programme


- Demonstrable skills and experience of General Medicine or a medical sub specialty
Clinical Skills

- Advanced Life Support Certificate from the Resuscitation Council UK or equivalent by the intended start
date.
Demonstrate evidence of being able to
- manage/prioritise time
- manage information effectively
- prioritise clinical need
- organise ward rounds
- Ability to maximise safety and minimise risk
- Monitor developing situations and anticipate issues
- Evidence of involvement in an audit project, a quality improvement project, formal research project or other
