RE: AUTHORITY FOR ROAD CUTTING ON GREAT NORTH ROAD

Reference is made to the above captioned matter and your letter in which you
requested for permission to carry out road cutting on the Great North road.

I wish to inform you that your request to carry out replacement and repairing of
a water pipe on part of the road reserve has been granted for the road as per
schedule.

Nevertheless, permission is based on the following conditions:

i. Lusaka City Council reserves the right to enter upon the land, if need arise,
for any future road works or installation of other services at no cost to the
Council.
ii. Reinstatement must be done upon completion of the works failure to which
you will be charged fivefold the cost of reinstatement.
iii. You make arrangements to control and direct traffic during the period of
works which should also include road signage. Furthermore, all detour
routes should be identified and prepared before road cutting works begin.
iv. Liaise with other stake holders/utility entities that have/may have their
network underlying along the site you propose.
v. You are required to inform our Engineering Services Department as soon
as you are ready to commence works so as to ensure that standards are
adhered to.
vi. You are therefore required to pay required to pay a total of Sixteen
Thousand Seven Hundred Kwacha, Fifty-Six Ngwee, K16,700.56, which
shall be deposited directly to the account details given below. Note that
failure by the applicant to provide proof of payment deems this authority
invalid.
