Miranda was already employed by GOMB when she met and married by son, Tanner. I do not
directly supervise Miranda and DHRM has confirmed that because her direct supervisor is Phil
Dean, the State of Utah nepotism rules have not been violated.

I recently co-authored a book entitled Stop Decorating the Fish. The book outlines in story form,
some of the concepts and ideas for improving performance through use of the Theory of
Constraints (TOC). The book can be purchased through Amazon. Any proceeds I may receive
from the book will be donated to the National Federation of the Blind. While I am employed by
the State of Utah, I will not receive any proceeds from books sales.

during the past five years in which you have engaged for the purpose of influencing the passage
