Dear Sir/Madam;

We wish to renew Ms. Maribel Sinaya of our company, Horizon Manpower and Engineering Services
for our Client Crownford General Corporation (Organic) as promodiser. He/She will be responsible
in selling, promoting and refilling the product of the client.

In her stay with Gaisano Mall, Ms. Maribel Sinaya. is directly employed by Horizon Manpower and
Engineering Services who shall be responsible for her salaries/wages/benefits as provided by the law.
He/She shall not be considered as an employee of your company but our own employee. He/She will be
under our control and supervision and shall not perform duties of your regular employees.

Ms. Maribel Sinaya will be assigned to one of your Outlets/Branches at Gaisano Mall, Digos City
starting October 10, 2019 to March 10, 2020. He/She will abide/follow the rules of our agency,
which are aligned with the general rules and regulations set forth by your company.

Likewise, in cases where he/she commits/violates any of the company rules of your store, you as our
client, may coordinate with us, so we can take action and impose proper disciplinary action. While he/she
is being investigated, we guarantee to provide replacement for him/her. In case of pilferage and other
crimes that may result to damage your company and if proven guilty during investigation, our company
has the sole responsibility to return and pay off charge the loss incurred as a result of the pilferage
investigation and/or to any similar incidents such as mention above.
