Drop Object Prevention Checklist

This checklist is provided as a tool for users to quickly perform a site analysis for any work at height. It is not to be used as a
substitute for conducting a full hazard analysis. It was developed to prevent falling objects which can be a major source of potentially
serious injuries or fatalities. All steps should be discussed and completed by Work Team Leader during JLA meeting.

2 Are crew members working at different platform maintaining proper communication with
each other?
3 Is there any gap or hole on the scaffold platform where tools and materials could fall
through?
4 Are all dropped object prevention tools (canvas bags, tools lanyard, orange netting etc) in
place and inspected?
5 Is orange netting required to install from floor to top rail of scaffold?
6 Does the tools lanyard have adequate strength for holding tools or hand tools being used?
7 Does material lowering and raising task require rope, Gin wheel, carabineer in addition to
canvas bags?
8 Is barricade erected with sufficient distance and signage posted on potential hazards?
9 Is the barricade blocking any emergency equipment (fire extinguisher, eye wash etc) or
emergency evacuation routes?
10 Are all work crew trained on tools and materials passing technique (twist method)?
11 Do the crew members know that concerned personnel should be notified for any dropped
object?

6 Any non essential tools or materials stored on scaffold deck?
7 Is there any improperly stored loose item that has been covered and hidden underneath other
materials on the work platform?
8 Is safety netting installed underneath the scaffold for employees and equipments below?
