AFFIDAVIT

I, Margarito G. Santos Jr, Filipino, of legal age, married, and a resident of 21 Narra St. San Luis, Malolos,
Bulacan, Philippines, after being sworn to in accordance with law, depose and state:

1. That I am the President of Bagong Hugas Corporation, a corporation incorporated and duly registered
under Philippine laws.

1.That Tiger Gaz Corporation is the registered owner of a certain motor vehicle that is more particularly
described as follows:

Body Type: Truck Chassis No.: JL6JAB6HXJK1083945

Series: CANTER EE71 C/C Plate No./ Conduction Sticker: HN9240

2. That the original body type of the said motor vehicle was " TRUCK ";

3. That the original series registered of the said motor vehicle was “CANTER EE71 C/C”;

4. That to have more use of the said vehicle in my business, I have decided to change the body type of
the said vehicle from " TRUCK " to " STAKE ";

5. That to have more use of the said vehicle in my business, I have decided to change the series of the
said vehicle from " CANTER EE71 C/C " to " PIPE RAILINGS ";

4. That the changes in the body type and the series are not intended for any illegal or unlawful purpose
but solely due to the foregoing reasons;

For delivery of sacks of rice and other goods.

5. That I am executing this Affidavit to attest to the truth of the foregoing and in support of the
application for the change of body type of the above-described motor vehicle on its Certificate of
Registration.
