which I am required to submit a reply relative to aforesaid Affidavit
concerning complaint __________ Grave Misconduct, Conduct
Unbecoming of a Government Official against _________;

2. I am confirming my statement executed and filed before the

3. Relative to the statement of __________ that he is a business

partner of _________, petition to show proof of his declaration that

applealing for the examination of his duly subscribed Statement of

2019 to forcibly break/open the padlocked steelgate door of the office
space and removed the notices placed thereat; in meddling during
my altercation on ______, 2019 with ________, likewise
declaring/disguising himself as the husband of ______, and also not
respecting the invitation of the Barangay representatives on
_______ to settle the problem at the Barangay Hall are acts in
contrary to the norms of conduct of public officials and employees;

6. I categorically state that _____ has no personality to

meddle/intervene with the problem of Contract of Rent between me
and ________ and that his behavior is contrary to Republic Act No.

for Public Officials and Employees, for bragging, coercing,
harassing/threatening, uttering slanderous words, by using his
position and likewise enriching himself by continuously using my
office space in transacting his alleged insurance business despite
demand to vacate the premises;

7. I am submitting for your review to this Honorable Committee a copy

of the video footage recorded during the altercation on ________to
proved the veracity of my accusation against _______ and to deny
his statement of being “imaginary exaggerations”.
