ACCIDENT OR INCIDENT

Accident or incident is section that story about before, during and after the one
particular event as accident happened. Based on this report, our group we will cover Health
and Safety Engineering course about a movie that we selected which is The Pressure. This
movie was released in 2015. This movie is about the 4 men from oil company was tasked to
help fixing the pipeline at underwater at depth of 200m below surface.

Before the accident occur, the oil company of Vaxxilon that placed at the Indian Ocean
off the coast of Somalia, detected the underwater pipeline has damaged that need to be fixed
as soon as possible. The chief of the oil company send 4 men to fix it but they was noticed that
the weather that night is not suitable and could be hazard to them. Even though, they still going
to dive that night because of the cost. The longer the time they delayed the fix, the more profit
they wasted. Hence, they decided to go that night with a diving bell, 2 diving suits and a ship
that support the bell to the underwater. Reflect to the safety of procedure, they should not go
dive at that night because of weather shows storm will coming to them at that night. In addition,
go diving during night is not suitable timing because lack of vision and consuming time for the
men who stay at the ship to call for help or doing something to help the diver if anything
happen.

Suddenly, the cable that holding the diving bell was break because of the storm. The 4
men was trapped in the diving bell with lack of oxygen supply under a deep water. The pressure
from the underwater make them distress and one of the crew took a short decision to wear one
of the suit to find a wrecked ship for oxygen supply but suddenly, he experienced an
hypothermia and enter a delusional world. Then, they men left in the bell need to cut the oxygen
tube for one of his crew for sake of their life. They keep contacting the men who on the ship
which they were lost because of cable and the storm. For safety management, they need to be
calm if this kind of situation and need a group decision making to rate and rank the best action
to take for sake of their live together. The chief of oil company also need to have an extra
backup plan, such as an extra diving bell or extra manpower on the ship for them because they
already knew that storm will coming and something could be happen to them.

The remaining men was finally contacted with Chinese fishermen that struggling to
communicate with them to ask for help. Suddenly, they connected with the local navy and ask
for help but unfortunately, their exact location under the sea was unable to be detected by the
radar. The navy was unable to detect them because their position was too deep under the water.
Then, one of them decided to bring the locator as near as he can for the navy can detect their
location but suddenly, there were a large group of jellyfish which the stung from them can
bring to death and that is what happen to a man who sacrifice to bring the locator near to the
surface. Reflect to the safety management, working at night is not very suitable for humankind
because lack of vision especially underwater where the world full of risk for humankind.

One of the remaining men need to be sacrifice because suit that left only one for them
to wear to reach to the surface. Their oxygen supply was reaching the red zone where they will
die if they do not do something to reach at the surface. The rescued man was badly internal
injured after reaching to the surface and the other man swimming trying to reach to the surface
but unfortunately he die because lack of oxygen and he drown.

As conclusion, the safety management is crucial even for sake of profit loss. This is
because it can bring a bad reputation to the higher management and the company. If the news
of this accident spread out to the world, they will be bankrupt of no partnership want to do
business with them anymore because of greedy attitude that put safety aside. From this
accident, they killed 3 professional men and 1 badly injured. Other than that, they loss a diving
bell and diving suit which it is costly.
