The video shows a welder using oxy-fuel cutting(OFC) to cut a supposedly empty fuel
tank, however the fuel residues in the tank cause an explosion that kills the welder and
damages other people who closely watched the work.
DANGER: Remaining gases from the tank
RISK: Damage or death by explosion
To weld a closed container, cleaning must be done, after degassing the tank and
circulating air using ventilatirs and extractors. Even when working, the air must continue
to circulate to avoid the concentration of flammable gases. Non-phosphorus solutions
should be used for washing and cleaning the tank.

The video shows a fireman explaining the accident that occurred when a welder tripped
and fell from a great height after releasing the safety harness to move freely.
DANGER: Work at height
RISK: Fall or death injuries
When welding at height, the necessary safety equipment should be used to prevent falls.
Check that all these elements are in good condition and properly installed. Never, under
any circumstances, do you need to release the harnesses when the welder is still in
height.
