PETITION

COMES NOW, the Petitioner, and before this Reconstituting Officer thru the
Register of Deeds, very respectfully states that:
1. Petitioner is of legal age, Filipino, married to ABNO Y. REYES, and a
resident of 143 Pagmamahal, Calasiao, Pangasinan where summons and
other legal processes may be served;

2. Petitioner is the registered owner of a parcel of land situated in the Barrio

of Pagdurusa, Municipality of Calasiao, Province of Pangasinan,
covered by and described in Transfer Certificate of Title No. T-127961
containing a total area of 1000 Square Meters, more or less, and with
metes and bounds as stated in the certified original photocopy herein
attached as “Annex B” and made an integral part of this petition;

3. The original of the aforesaid Transfer Certificate of Title was lost when
petitioner and her family’s humble dwelling was ravaged by Super
Typhoon “Malupit” last February 14, 2019. The Affidavit of loss is
hereby attached as “Annex C” and made an integral part of this humble
petition;
 4. Upon further inquiry to the Office of the Register of Deeds of Lingayen,
Pangasinan, circumstances revealed that the original file copy of said
office was destroyed as an unprecedented fire devoured their files last
January 01, 2019. Revealing further, that the Land Registration
Authority located in Quezon City had no copy of the aforementioned
Transfer Certificate of Title;

5. Petitioner is in actual possession of the subject parcel of land;

6. There was no deed of other instrument affecting the property that has
been presented for registration or if there be any, the nature thereof, the
date of its presentation, as well as the names of the parties and whether
the registration of such deeds or instruments, is still pending
accomplishment;

7. The subject property is free from any and all liens and encumbrances of
whatever nature.

8. Submitted herewith are the following papers in support of this petition:

a. Certified Original Photocopy of TCT No. T127961, in three (3)

copies hereof;

b. Certified true copy of Real Estate Tax Clearance representing

full payment of at least two (2) years prior to the filing of the
petition;

c. Three (3) certified true/copies of the latest Tax Declaration;

d. And other pertinent documents.

PRAYER
WHEREFORE, the petitioner very respectfully prays that the
Original/Transfer Certificate of Title No. T-127961 that is lost or destroyed be ordered
reconstituted on the basis of the owner’s Certified Original Photocopy of Transfer Certificate
of Title No. T-127961 and in accordance with Republic Act No. 6731.
