SUBJECT: Delegation of Authority for Dr. Herminigildo V. Valle, Undersecretary of

Health and Chairperson of COBAC D as Signatory to the Deeds of Sale of
Ambulance Type III Advance Life Support Vehicle under RC No. GOP
2017—12-0325

In the exigency of service, authority is hereby granted to Dr. Herminigildo V. Valle,

Undersecretary of Health and Chairperson of COBAC D, to sign as official representative of
the Department of Health, the deeds of sale of 128 units of ambulance type III advance life
support vehicle under RC No. 2017-12—0325.

This order shall take effect immediately.
