MEMORANDUM OF AGREEMENT

Know all men by these presents:

This Memorandum of Agreement ( “ Agreement” )

Is entered into by and between the following,

The Barangay Tinago headed by our Barangay Chairman as well as the Barangay council
are having an agreement about to put up the Health Post center address to Lower Tinago in front of
Zyryll Latorilla residence .The Chairman and the members of the councils are the FIRST PARTY,

The lower Tinago Elementary School ,A duly registered and recognized Lower Tinago
Elementary School with postal address at Barangay Lower Tinago, headed by Mr. RICHARD PADERNAL
as head teachers or principal of this school, a duly represented as officers in charge of its school area
hereinafter referred to as the SECOND PARTY.

Witnesses:

Whereas, the first party has established a health Post center for its Constituent of Barangay
Lower Tinago which was duly approved by the local Government unit ( LGU ).

Whereas, one of the objectives of the health post is to maintain the necessity of the
constituent of Barangay Lower Tinago and to maintain their health problem.

Whereas, The first Party has chosen the second party as one of the center place and good
venue nearest to the some houses, the health post purposes is to given for the people of Barangay
lower Tinago the easy consultation of their health problem.

Whereas, The second party has agreed to cooperate with the first party to the utilization of
the formers premises and facilities as an extension of the health post center.

Now therefore, for and in consideration of the foregoing premises the parties bind themselves
to the following terms and conditions,

OBLIGATION OF THE FIRST PARTY;

1. The first party shall provide the second party with the copy of the current guidelines of the
house hold of lower tinago.
2. The first party shall provide the second party with the list of the name of BHW who will
assigned to lower tinago.
3. The first party undertake to inform the second party if we have some other information from
the main office or from the local Government unit.
4. The first party undertake to withdraw its endorsement in favor of any constituent of disciplinary
infraction and after due to investigation will proved to be unfit to continue their services.
 5. The first party agree not to use the Barangay address of the second party in any publication or
advertising except for the Barangay recruitment materials or school materials.

OBLIGQTION OF THE SECOND PARTY :

1. The second party shall send a formal letter to Barangay if we have received some information
for health activities .
2. Before the activities of the health, the second party will conduct and orientation meeting to the
constituent of lower tinago and the Barangay council.
3. The second party have the right to control the members or the persons who well assigned to
the health post.
4. The second party is primarily responsible for the actual supervision and control for any necessity
of the health post center.
5. The second party shall coordinate with the Barangay for any concerned involving the
constituent of lower tinago .

By; signing below, both parties consent to the terms and conditions of this agreement.
