2. Download this Open Canvass Form from the PhilGEPS website before the closing date.
3.Bids/quotations may be submitted thru fax, email or directly to the BAC Secretariat Office on or before the
deadline of submission of bids and requirements.
4. Suppliers are required to submit the following prior to the notification by the BAC of the award: a) Valid and
Current Mayor’s/Business Permit; b) Professional License/Curriculum Vitae (Consulting Services); c) Valid and
Current PhilGEPS Registration Certificate/Number; d) Philippine Contractor’s Accreditation Board License
(Infra); e) Income/Business Tax Return (for ABC’s above Php500,000.00); and f) Omnibus Sworn Statement (for
ABCs above Php500,000.00).
5. Bids should be valid for 45 days counted from the deadline of submission.
