In the reference cited above the Assistant Divisional Engineer, Operation Sub Division, Ganapavaram
has submitted the representation of Smt. G.Padma, SWG to retire on medical invalidation. The
representation is here by returned to attend the following remarks

1.Smt.G.Padma has applied for medical leave from 29.4.2016 with her signature on the application.
But in the representation to retire on medical invalidation received with finger print ( Nisaani) of the
incumbent. If She is unable to sign on the application the finger print ( Nisaani) should be accompanied by
witnesses along with their identity proofs

2. Smt. G.Padma has submitted the doctor certificate issued by Sri.G.Satyanarayana Raju, SRI
SRINIVASA SATYA SAI MEDICAL CENTRE, Bhimavaram. But does not containing the period of treatment and
date of certification

3. The representation does not containing the permanent address of the incumbent for further
correspondence in this regard

Hence, it is instructed to resubmit the above duly attending the above remarks at an early date

It is to submit that Smt. Guttala Padma, ( ID No 1028557) who has been working as Sweeper cum
Gardener in APEPDCL has represented to accord permission to retire on medical invalidation since she has
been suffering from “Diabities/HTN/and paralysis with one side i.e right side upper limb and lower limb”.
The copy of the representation is here with enclosed along with Medical certificate issued by
Sri.G.Satyanarayana Raju, SRI SRINIVASA SATYA SAI MEDICAL CENTRE, Bhimavaram in original

In this connection , it is requested to cause necessary examination as she has proceeded on leave on
medical grounds with effect from 29.4.2016 duly stating that she cannot discharge the duties being a
Sweeper cum Gardener and to arrange to provide the medical report at an early date to take further action
in this regard please

It is to submit that Smt. Guttala Padma, ( ID No 1028557) who was worked as Sweeper cum Gardener
in APEPDCL has represented to accord permission to retire on medical invalidation since she has been
suffering from “Diabities/HTN/and paralysis with one side i.e right side upper limb and lower limb”.
Accordingly Smt. G.Padma, SWG was referred to medical examination by the District Medical Board

In the reference 4th cited above the medical examination report inrespect of Smt. G. Padma received
from the Medical Board, WG.Dt.,. But the office seal (Round Seal) not available on Medical Board report.

Hence, it is requested to arrange to provide the office seal( Round Seal) on the medical board
certificate which was enclosed here with in original and to retransmit the same to the undersigned at an
early date please
