PROMISSORY NOTE

I, xxx, Filipino, of legal age, and a resident of xxx Pampanga, promise to pay,
jointly and severally, to the order of xxx RAMOS, Jr., of legal age, Filipino and
residentsof San Vicente, Calasiao, Pangasinan, the total amount of Four Hundred
Fifty-Nine Thousand (Php459,000.00), Philippine Currency, payable within a period of
ONE (1) YEAR, from and after date hereof, payable monthly in the amount of Thirty-
Eight Thousand Two Hundred Fifty (Php38,250) Philippine Currency, per month, and
shall be paid every 8th of the month starting from November 2019 to December 2020.

That in case of failure to pay the said monthly amount within the period
stipulated, the total amount shall forthwith become due and payable without need of any
prior demand whatsoever;

That in the event that this note is placed in the hands of an attorney for collection
or for the enforcement of its terms hereof, I further bind myself, to pay the total amount
due and payable as attorney’s fees, aside from other legal costs and expenses;

That the LEASE/PURCHASE AGREEMENT, over a house and lot located at xxx,
registered in the name of xxx and entrusted to xxx by way of Waiver of Rights, and a
Special Power of Attorney both duly executed on 06 July 2017, shall be left in the
possession and custody of xxx during the period of this loan. The same shall be
returned to me upon full payment of our said obligation. In the event that I declare said
LEASE/PURCHASE AGREEMENT as lost, whatever my remaining obligations shall
become due and demandable. Copies of the Lease/Purchase Agreement, Waiver of
Rights, and Special Power of Attorney, are hereto attached as Annexes “A”, “B”, and
“C”, respectively;

Venue, in case of litigation shall in the proper Court of Angeles City, Pampanga,
or at the option of the payee;
