This application template should be completed and submitted to CPA Australia by a new higher education provider seeking
professional accreditation for a new accounting program or for a new twinning arrangement. Evidence documents should
accompany the completed template. Electronic submissions are preferred and need to be addressed, along with the supporting
documentation, to CPA Australia via: accreditation@cpaaustralia.com.au
