10-04-18 WRITTEN COMMENDATION

Sergeant Creigee Coleman commended you for your actions on

September 18, 2018. Officer Justin Hayes observed a disabled vehicle
that drove away from him when he approached. Officer Hayes relayed
a description and that the driver was erratic and may have a medical
condition. You positioned your cruiser in front of the car in order to
keep it from crossing into oncoming traffic. The vehicle slowed at
which time Officer Ronald Miller ran next to the vehicle and
subsequently jumped in and was able to get the driver, who was
suffering from a Diabetic incident, to stop the car.

08-07-18 WRITTEN COMMENDATION

You were commended by Sergeant Michel Pauley for your actions on July 26, 2018, after
multiple suspects gained entry into Sandy’s Towing and stole five vehicles. Later that
morning, you spotted one of the vehicles 1931 Bender Avenue where three males were
attempting to cover the vehicle with a tarp. The males ran and you provided a description
and direction of flight. Officer Christopher Savage quickly apprehended two of the
suspects while Officer Campbell apprehended the third suspect. Detective Mike Baker
located another of the stolen vehicles in the 3200 block of Hoover Avenue and observed
two suspects exit the vehicle. You subsequently apprehended one of those suspects.

5-28-18 WRITTEN REPRIMAND

Sergeant Ryan Halburnt commended you for your actions on May 23,
2018, while on routine patrol, Officer Michael Schwartz observed a
Pontiac Grand Prix with excessive window tint. As he started to pull the
car over, it suddenly pulled into a driveway and the passenger fled
from the traffic stop. He requested back-up and secured the remaining
individuals. You responded as back-up and assisted in locating two
loaded firearms in the vehicle. After the passenger that fled returned
to the vehicle, you checked the area where he came from and found
another loaded firearm and 20 grams of heroin. The investigation
revealed the firearms were stolen earlier that morning in Mason, Ohio.

4-19-18 WRITTEN COMMENDATION

Sergeant Creigee Coleman commended you for your actions on April

19, 2018, when you were dispatched on a wanted call at 1540
Wesleyan Road. You were able to determine that the suspect had a
nationwide warrant for Aggravated Robbery. After setting up a
perimeter around the residence, you tried to call the suspect out,
however, she refused. Utilizing the suspect probation officer, you
developed a rapport with the suspect and were successful in coaxing
her out of the house and to surrender peacefully.
10-01-14 WRITTEN COMMENDATION

Lieutenant Wendy Stiver commended your dedication to your duties and relentless
pursuit of criminals that resulted in the arrest of an extremely dangerous violent felon.
The incident occurred on August 23, 2014 when you and Officer Jeffrey Spires were
dispatched don a shooting in the area of Strasburg Drive and Liscum Drive. Officer
Spires arrived first and found the victim lying in the street with multiple gunshot wounds.
You arrived and block the street with your cruiser and you both secured the scene. Your
investigation on scene resulted in the arrest of a named suspect, David Henderson, just
five days later.

1.12.6 LETTER OF APPRECIATION (e-mail from Lt. Carper)

DMHA Task Force received Certificates of Appreciation from DMHA and from
Mayor Rhine McLin during a ceremony yesterday. This was in response to the
hard work and dedication these task force members have to the residents of
DMHA property as well as the tremendous amount of activity generated by this
small unit.

11.14.5 WRITTEN COMMENDATION

Sergeant Steven Abney commended you for the arrest of Demitres Ward, a Felonious
Assault suspect possibly involved in a recent homicide. On November 10, 2005, Ward
led you and other officers on a dangerous pursuit until he finally fled from the vehicle on
foot into Wolf Creek. Ward then fled into the storm sewer system with you and Officer
Herb Rogers giving chase. As Ward fled he could be seen holding his waistband
indicating he was probably armed. You and other officers entered the sewer system and
found Ward hiding inside. Ward was booked on a host of Felony crimes including
Aggravated Robbery and Burglary. In addition he was charged with Felony Fleeing and
Eluding.

7.29.5 LETTER OF APPRECIATION (e-mail from Lt. Robert Chabali)

On July 29, 2005 The Dayton Police Department received a Letter of Commendation
from Agent Herb McGuire, Craven Bail Bonds, Springfield, Ohio, telephone #1-740-288-
2222, reference the prompt and courteous response provided by Officers Melissa Orick,
Tim Liddy and John Mallot during the arrest of Garrett Donovan Kendall on July 28,
2005 at 2122 Germantown Pike. Mr. McGuire notes that the professionalism, timeliness,
and willingness to respond to the situation serves as a great reflection on all Officers of
the Dayton Police Department.

4.14.3 WRITTEN COMMENDATION

Sgt. Abney commended you for the apprehension of an armed suspect on April 11, 2003
at W. Third St. and S. Gettysburg Ave. on a high-risk felony stop. The skill,
professionalism and courage you displayed in this incident are to be commended.

8.4.5 WRITTEN COMMENDATION

Commended by Sergeant H. Kelly Hamilton for your aggressive police

work and diligent observations resulting in the apprehension of two
violent criminals. The incident involved the robbery of a small grocery
store located at 475 S. Broadway Avenue on July 22, 2005.
