STANDARD INSERTIONS TO HMO MOA

1. ACEMC-Bohol shall send its Weekly Statement of Accounts (SOA) to


Cooperative Health Management Federation one day after the end of each
week. Each weekly SOA shall include all the patient bills for the
Cooperative Health Management Federation members who received
services from ACEMC-Bohol as approved by the Cooperative Health
Management Federation, between the last weekly SOA and the current
weekly SOA. The aging of accounts receivable by ACEMC-Bohol from
Cooperative Health Management Federation shall be done on a per SOA
basis.
2. Each SOA shall be paid in full by the Cooperative Health Management
Federation within 30 days after date of receipt by the Cooperative Health
Management Federation.
3. ACEMC-Bohol shall entitle the Cooperative Health Management Federation
to prompt payment discounts according to the following schedule :
a. If a SOA is fully paid within 30 days after date of receipt by the
Cooperative Health Management Federation, ACEMC-Bohol shall
entitle the Cooperative Health Management Federation to a Prompt
Payment discount of 6 % of total net amount of the SOA.
b. If a SOA is fully paid between 31 - 60 days after date of receipt by the
Cooperative Health Management Federation, ACEMC Bohol shall
entitle the Cooperative Health Management Federation to a Prompt
Payment discount of 4 % of total net amount of the SOA.
c. If a SOA is fully paid between 61 - 90 days after date of receipt by
HMO, ACEMC Bohol shall entitle the Cooperative Health Management
Federation to a Prompt Payment discount of 2 % of total net amount
of the SOA.
d. SOA’s that are fully paid between 91 – 120 days are not subject to
prompt payment discount.
All earned prompt payment discounts shall be initiated by ACEMC-Bohol
through its own credit memos that shall be applied to the subsequent SOA’s
to the Cooperative Health Management Federation. The Cooperative Health
Management Federation shall not presumably deduct these discounts from
their payments to ACEMC-Bohol.
4. Any SOA or portions thereof that remain unpaid after 120 days after date of
receipt by the Cooperative Health Management Federation shall be subject
to surcharge of 5 % per month or fraction thereof until fully paid or settled by
whatever means.
5. Any error in the charges shall be deducted by ACEMC – Bohol in the form of
credit memo that shall be applied to subsequent accounts of the Cooperative
Health Management Federation.
6. ACEMC-Bohol shall have the right to suspend services to members of
Cooperative Health Management Federation when the total amount payable
by the Cooperative Health Management Federation to ACEMC Bohol
reaches PHP 250,000.00 or when there is any SOA or portion thereof that
remains unpaid after 90 days after date of receipt of SOA. When any of the
two conditions that will warrant suspension of services as stated in this
section is reached, ACEMC-Bohol shall issue a formal notice to the
Cooperative Health Management Federation through electronic mail from the
CFO of ACEMC-Bohol to the designated officer of the Cooperative Health
Management Federation, followed by a formal notice in hard copy within 2
days. Services shall be resumed when all SOA’s that are aged more than 90
days are fully paid and the total accounts payable by the HMO to ACEMC-
Bohol falls below PHP 125,000.00.

7. If and when the suspension of services due to any of the two reasons
cited in section 5 lasts for forty five (45) days after suspension date, the
total accounts payable by the Cooperative Health Management
Federation to ACEMC-Bohol including applicable surcharges , shall be
sent to the legal counsel of ACEMC-Bohol for litigation processes. All
litigation expenses including legal and court fees shall be for the account
of the Cooperative Health Management Federation. Henceforth ACEMC-
Bohol shall have the option to terminate this agreement upon issuance of
a written notice.

8. All shall consultants of ACEMC – Bohol shall be considered accredited


under the umbrella accreditation with the company and may charge their
professional fees higher than schedule attached to this agreement for as
long as the patient agrees to pay directly to the doctor the amount in
excess of the rates provided in the schedule attached herein. All check
payments by the Cooperative Health Management Federation for the
professional fees of the doctors shall be under the name of ACEMC –
Bohol, which will in turn remit these to the concerned doctors as soon as
paid by the Cooperative Health Management Federation to ACEMC –
Bohol.

9. In case of any litigation arising out of the Agreement, the venue shall be in
Tagbilaran City, Bohol where the services offered.
