AFFIDAVIT OF DISCREPANCY

I, APOLONIO TOLENTINO y DELA CRUZ, Filipino, of legal age, single, with

postal address at Purok 3, Brgy. Macabling, City of Sta. Rosa, Laguna, after
having duly sworn on oath deposes and alleges THAT:

1. I am the ACCUSED in Crim. Case No. 31226-B for Frustrated Homicide

before the Regional Trial Court of Biñan, Branch 25;

2. The name indicated in the title of the above-mentioned case is PAUL

DELA CRUZ TOLENTINO, which is erroneous because my Paul is merely
my nickname;

3. For the above-entitled case, my brother posted a cash bond in the

amount of Twenty-four Thousand Pesos (P24,000.00), likewise under my
nickname;

4. my full name is APOLONIO TOLENTINO y DELA CRUZ, as per my proof of

identity, being my Phil health ID No. 19-090512203-0, a copy of which is
hereto attached as Annex 1;

5. I pleaded guilty to the lesser crime of Attempted Homicide, and as

penalty, have undergone and completed my probation;

6. I am executing this affidavit for the purpose of obtaining the release of

the cash bond posted in relation to the said case.

In truth of all the foregoing, I have hereunder affixed my signature this

27th day of June, 2019, in the City of Biñan, Laguna.
