INITIAL DATA BASE FOR FAMILY ASSESSMENT

Family Name: V family

Household no.: 5
I. INTRODUCTION
The family nursing process is utilized by the nurses in order to address the health needs and
problems of the client. Like the nursing process it also utilizes the nursing assessment which is
the first major phase of the nursing process. After using these said materials in order to create a
family nursing care plan for our chosen family in the community. Which in this case is the
family V
Family V is our chosen family that will receive our nursing interventions throughout the
utilization of our plan of care. They live in phase 1 Bukid at Ilugin, Pinagbuhatan, Pasig City.
They are a family of 6 plus 1 with the husband, their main occupation is cooking ang selling corn
on cobs coming from suppliers and orders. They were our chosen family because I think they are
the one needing a more immediate action and positive reinforcements compared to the other
family in our area. We identified 3 family problems through proper scoring and prioritization and
upon identification of these said problem we have chosen this family. The life in Ilugin is not
that great but even considering the amount of hardship in their everyday life this family is even
more marginalized compared to the other families around the area. As nursing students, we
would like to the something for them and learn more things that will help us in certain situations
in the future when we encounter more people in line with them, we will be able to act
accordingly and provided them the best possible nursing care the world has to offer.
