PROMISSORY NOTE WITH MORTGAGE

FOR VALUE RECEIVED, I/We the undersigned jointly and severally promise to pay to
the order 786 Chandi Lending Corporation with office address at #182 Golden Shower Villa
Ofelia, Cabanatuan City the sum SIX THOUSAND PESOS ONLY (P6,000.00) Philippine
Currency within a period of 40 DAYS excluding Saturday and Sunday divided into ONE
HUNDRED FIFTY PESOS ONLY (150.00) DAILY/weekly/semi-monthly/monthly with
amortizations as follows:

ONE HUNDRED FIFTY PESOS ONLY (150.00) payable DAILY and the remaining
balance in equal successive _______ installment(s) of

Failure on our part to comply with any of the above-stated amortizations will render the
entire obligation due and demandable together with all the interests and penalties that may have
accrued without prior notice. Interest not paid when due shall be added to become part of the
principal and shall likewise bear interest at the same rate as the principal, compounded monthly.

The creditor may accept partial payments with reservation to institute the proper recourse
against each and all endorsers who are hereby waiving the right to demand presentment and
notice. I/We jointly and severally agree that payments will be applied first to interest, then to
penalties and other charges, and then to principal.

As security for the payment of the above obligation, we hereby convey by way of
Chattel/Real Estate Mortgage to the CREDITOR the property particularly described set forth in
the list appearing at the back hereof which we are the absolute owners and the one in possession
of the same, free and clear of any lien, encumbrance and adverse claim.

It is hereby agreed that the encumbrance created by this mortgage shall apply not only to
the mortgaged properties but also to all properties in our premises whether brought to the
premises before or after the execution of this agreement.

In case of default, we hereby agree to deliver the subject properties on demand of the
CREDITOR who is likewise authorized to enter the premises where the property is situated
without any order from the court or any other power or permission than that herein granted. All
expenses incurred in locating bringing the mortgaged property to the CREDITOR shall be at our
expense which shall be added to the sum demandable and the CREDITOR shall have the option
of selling the property either at public or private sale.

Should we fail to deliver the mortgaged property to the CREDITOR upon demand, the
same shall be considered admission and confession that we removed, sold or pledge the property
mortgaged.

The CREDITOR shall have the right to purchase the mortgaged property or any part
thereof at public auction held for the purpose of foreclosure of this mortgage judicial or
extrajudicial and to have the amount of its bid applied to payment of the obligation secured by
this mortgage.

Upon default of the payment and this note is endorsed for legal action for collection, we
shall, in addition, pay 25% of the amount due as attorney’s Fee which sum shall in no case be
less than P2,000.00 aside from the legal cost and expenses of litigation. All actions arising from
or connected with this note shall be brought only at the proper courts of Cabanatuan City or at
the place of execution of this note, at the sole option of 786 CHANDI LENDING
CORPORATIONS.

I/We certify that the provisions and contents of the foregoing document were fully and
severally explained to me/us by the 786 CHANDI LENDING CORPORATIONS thru its
representative and I/we fully understood the same.
