ID Requirements for Garda Vetting

When a Garda Vetting Application Form [NVB1] is submitted to St Vincent's Hospital, Fairview for processing, two
forms of Identification [ID] must be presented in person.

The two forms of ID that are required are:

1. Photo ID which must be one of the following:


a. Irish Driving Licence or Learner Permit
b. Irish Public Services Card
c. Passport
d. Irish Certificate of Naturalisation
e. Garda National Immigration Bureau Card [GNIB]
f. National Identity Card [EU/EEA/Swiss Citizens]
g. Employment ID

2. Proof of Current Address which must be one of the following and cannot be more than 6 months old:
a. P60, P45 or Payslip showing home address
b. Utility bill – Gas, Electricity, Television or Broadband only [PLEASE NOTE: Mobile Phone Bills are
not acceptable]
c. Bank/Building Society or Credit Union Statement
d. Correspondence showing home address from:
i. An Educational Institution/SUSI/CAO
ii. An Insurance Company regarding an Active Policy
iii. A Bank, Credit Union, Government Body or State Agency
e. Letter from Employer [within the last 2 years]:
 Confirming name and address
f. National Age Card
g. Membership Card from:
 Club, Union or Trade, Professional Bodies
 Educational Institution
h. An Affidavit confirming current address witnessed by a Commissioner for Oaths**

PLEASE NOTE: The Garda Vetting form which you complete only covers addresses in the Republic of Ireland
and Northern Ireland.

If you have resided in countries outside of the Republic of Ireland and Northern Ireland for a period of 6
months or more, it will be mandatory for you to obtain a Police Clearance Certificate from those countries
stating that you have no convictions recorded against you while residing there. You will need to provide a
separate Police Clearance Certificate for each country you have resided in.

**An affidavit is a written sworn statement of fact voluntarily made by a person. It is a document that sets out in paragraph
form the evidence that the witness wishes to give. Affidavits are usually written and prepared by a solicitor or a barrister after
having obtained all the necessary information from the witness. The wording used in the affidavit will depend on the
circumstances of the case. Your solicitor can give you more information on the wording that will be used.

When the affidavit is ready, the witness must go before a Commissioner for Oaths. The Commissioner for Oaths will check
that the person swearing the oath has read the affidavit and fully understands the contents. The person will be asked to raise
the Bible and to repeat the words of the oath. If the witness does not wish to swear an oath on the Bible, he or she may make
an affirmation. He or she will then sign the affidavit. The Commissioner for Oaths will verify that the affidavit was properly
sworn by completing a jurat on the affidavit.
E-Vetting Process

Step 1
The Vetting subject manually completes and forwards to the Liaison Person in St Vincent’s Hospital, Fairview the
Vetting Invitation Form [NVB 1] and provides proof of identity.

Step 2
The Liaison Person in St Vincent’s Hospital, Fairview validates proof of identity and sends the Vetting subject an e-
mail with a link attached inviting him/her to complete a Vetting Application Form.

Step 3
The Vetting subject completes a Vetting Application Form online and submits it to the Liaison Person in St
Vincent’s Hospital, Fairview.

Step 4
The Liaison Person in St Vincent’s Hospital, Fairview reviews the Vetting Application Form and submits it to the
National Vetting Bureau.

Step 5
The National Vetting Bureau processes the application and forwards a vetting disclosure to the Liaison Person in
St Vincent’s Hospital, Fairview.

Step 6
The Liaison Person in St Vincent’s Hospital, Fairview reviews the vetting disclosure and as soon as is practicable
and provides a copy of the disclosure to the Vetting subject if requested.
