1. Project organisation for health and safety control

This risk assessment and method statement is for employees and sub-
contractors working for Brighouse Patent Walling Systems Ltd (BPWS). It
relates to the way in which pile cropping is to be carried out, in line with the
company’s Health and Safety Policy.

All operatives must be fully trained in the work they are doing. They can
contact Daniel Cryer at any time with regards to any issues that may arise.

All pile croping will be overseen by Daniel Cryer. All employees and sub-
contractors report to him and are expected to report any health and safety
issues immediately.

All employees and subcontractors will receive a copy of this risk assessment
and method statement and will work in accordance to it.

3. Method Statement
HARD HAT, SAFETY BOOTS AND HIGH VISIBILITY JACKETS
MUST BE WORN AT ALL TIMES
 All personnel must be familiar with site safety rules, the traffic
management plan and accident procedures.
 After receiving instruction about works to be carried out, personnel will
proceed to the relevant work area.
 Personnel must refer to the risk assessment and ensure that all current
control measures are in place and continue to be in place for the duration
of the work task.
 Areas of work are to be kept clean and free from debris at all times both
during works and on completion of work tasks.
 On completion of work all tools and unused materials are to be returned to
the appropriate stores. The work area is to be left tidy and any waste
materials or debris placed into appropriate skips provided by BPWS.
 The area will be assessed to ensure a safe route to execute task is
possible.
 The correct PPE will be worn: high visibility jacket, steel toe capped boots,
hard hat, gloves, goggles and ear protection
 Correct procedures for operating a Stihl saw will be followed and only
those trained in its operation will do so.

The main hazards to your safety and health are;


a) Falling from height
b) Injury from incorrect Manual handling.
c) Injury from slips trips and falls.
d) Contact dermatitis from exposure to wet concrete and dust.
e) Disease from standing/stagnant water.
f) Injury from the incorrect actions of other contractors on site.
g) Injury to members of the public during operations.
h) Injury from machine hazards

Preventative Measures that must be taken;


a) Employees must be “competent” to carry out the task.
b) Employees must NOT carry out this task alone
c) Barriers erected at entrances and around the work area if deemed
necessary by the foreman to protect tenants.
d) Employees must not lift beyond your capabilities, get help if necessary.
e) Visitors and other members of staff are prohibited from entry unless
accompanied by competent person, all visitors issued with personal protective
equipment
f) You must read and be familiar with the Safety Data Sheets for any COSHH
that may be used these will contain information on first aid, fire fighting, and
accidental release measures.
Environmental Protection Measures you must take;
a) You must dispose of waste and spoil to the designated area or skip
provided for waste.
Quality Control;

Arrival on Site
Ensure your vehicle is parked in the designated parking area for that
particular site where you are going to conduct your work activity and not
parked in an area which may cause a hazard to others.

Vehicular and Pedestrian Routes


Ensure you adhere to any advisory and mandatory signs indicating speed
limits for vehicles and designated routes on site also ensure as a pedestrian
you keep to any designated routes as indicated by the Principle Contractor to
reduce the risk of pedestrians impacting with vehicles and items of plant
machinery.
Personal Protective Equipment
Ensure that you are wearing the correct PPE; Hi-visibility waistcoats
Protective footwear Adequate Head Protection
Further requirements for PPE for the application of task on site, to be worn as
and when required; Gloves Dust Masks Safety Goggles

Work Area (General Precautions)


• The work area is to be free of other trades working in the immediate area.
• All debris, rubbish & trip hazards are to be removed from the work area prior
to the work commencing.
• If other trades are working within the property they must be advised as to the
work activities you are about to commence.

Maintenance and replacement of personal Protective equipment


Every employee or self-employed person shall ensure that any personal
protective equipment provided to them is maintained (including replaced or
cleaned as appropriate) in an efficient state, in efficient working order and in
good repair that they have received adequate training with regards to the use
of any particular item of Personal Protective Equipment.
• Self employed persons are to ensure they purchase and maintain PPE in
line with the relevant risk assessments
• Employed persons are to ensure they look after and use any PPE provided
for them and report any defects etc to their supervisor immediately.

Welfare Facilities
Operatives will use the welfare facilities provided by BPWS to include toilet
facilities and Mess Canteen, the canteen facilities will be used for all meal
breaks and no breaks will be held in the work area, on any other part of the
site nor in vehicles (Private or company owned)

On Completion of Works
All work areas are to be left in a clean condition and all aspects of safety with
regards to the immediate work area and conduct a visual inspection by the
operative to ensure that the edge protection is in place and that there is no
risk to injury to themselves or others as a direct result of a fall from that area.
Ensure all other material which is used for your work is stored in a safe and
proper manner before departing from the property you are working in.
Report to the Site Manager on completion of the works and if required by the
site rules sign out before leaving

Pile cutting steel
A “safe zone” will be demarcated around the temporary pile excluding all
persons not completing removal of the piles.
The top of the pile will be cut around the perimeter with a petrol cut-off saw, as
close to the soffit of the GF slab as possible.
The piles must be cut to the correct height as specified by the site engineer.
Two men will undertake this task, one to hold the pile, the other to cut it.
The pile should be cut most of the way through then pushed over in a safe
and controlled manner.
The excess piling will be removed from area using the 360 excavator or the
forklift truck and placed in a suitable location away from the working area.

Pile cutting Concrete


When the driving of a concrete pile is complete, the next step is to connect the
top of the pile with the structure it is holding up. To do this, it is frequently
necessary to have the reinforcing bar protrude above the top of the pile. There
are two basic ways to accomplish this:

1. Fabricate the pile with protruding reinforcing bar or cable; or


2. To cut off the top of the pile in such a way as to leave the reinforcing bar or
cable exposed for connection.

To cut the pile we should use the following method

A “safe zone” will be demarcated around the temporary pile excluding all
persons not completing removal of the piles.
The top of the pile will be cut around the perimeter with a petrol cut-off saw, as
close to the soffit of the GF slab as possible.
The machine driver shall wear eye protection and ear defenders at all times
when breaking the pile.

1. Mark the correct cutting level using survey instrument and cut along the
marking with cutting disk at least 40mm into the core.

2. Break the pile using large jackhammers up to 300mm above the


marked level. Or use a Pile Breaker Machine supplied by Piling
Contractors. However, some machine can cut the pile reinforcement
during the breaking process that leads to additional work of splicing the
reinforcement with couplers.

3. Break the remaining 300mmof pile head by small electric jackhammers


and flatten it to the correct level. Chipping of small loose concrete to be
done manually by hammer and chisel.

Finally all pile reinforcement shall be cut to the required development length
and all reinforcement with insufficient development length shall be spliced with
couplers.

Using 360 machine for lifting
Under no circumstances will excavators be used for lifting persons as they
operate at speeds which make them totally unsuitable for the lifting of
persons.
Excavators designed for lifting loads/ object handling should have a rated
object handling capacity table inside the cab. If a rated object handling table is
not present then the machine should not be used for object handling.
The use of an excavator as lifting equipment poses additional hazards as the
slinger needs to be in the “danger area” adjacent to the arm/ bucket etc.
during the lift to attach and monitor the load during the lift. As such, clear lines
of communication must be kept between the operator and the slinger at all
times.
All excavators and their lifting accessories will be suitable maintained, have a
Thorough Examination completed and a certificate issued a minimum of every
twelve months.
Prior to any lifting operations with excavators, the appointed person will
ensure that a risk assessment is completed, ensuring that the machine is
capable of lifting the intended load, that lifting accessories are adequate and
free from damage and that ground conditions are suitable to allow the lift to
take place.
The appointed person will brief all persons present/ involved in the lift on any
significant findings of the risk assessment, the method statement and
operatives individual roles in carrying out the task.
Where special lifting attachments are used, they must be compatible with the
machine and its coupling methods to which they are being attached.
The excavator operator must always make use of the safety lever/ armrest
whenever the slinger is in the danger area to avoid moving the controls by
mistake.
The hazards associated with travelling with a suspended load, particularly the
presence of personnel adjacent to the machines direction of travel, must be
considered, assessed and recorded on the lifting plan.
All persons involved will wear hard hats, protective footwear and high-visibility
clothing as a minimum.
Lifts will not take place where there are persons working underneath the load,
for example in a trench, and the excavator operator will not move the machine
until he has satisfied himself that all persons, including the slinger/ signaller,
have moved themselves to a position of safety.
When attaching slings to the hooking device or lifting point, care must be
taken to ensure that the slings and their attachments are able to hang free at
all times, particularly when using a “D-Shackle” as it may limit rotation.

5. Perseverance Mills site information

Work place access and egress

Access to and egress from the site will be provided by the BPWS.
Access to and egress from the site will be assessed before starting work to
ensure it is safe.

Work place Lighting

The work is outside and natural light is suitable to do the task.

Plant & Equipment

PPE
All PPE provided by BPWS will conform to the relevant EN number and
be fit for purpose.

Electrical Equipment
It is not anticipated electrical equipment will be required for this work.

Lifting Equipment
The cut of piles will be removed using the forklift truck or the 360
excavator using suitable lifting slings or chains.

General Equipment
Stihl Saw. The Stihl Saw will be fit for purpose and regularly inspected for
defects.

Personnel Training/Certification
All personnel will be sufficiently trained and have the relevant qualifications to
carry out the task at hand

Training in the use of a Stihl Saw will be given.

Waste Management

All waste generated will be removed off site by the sub-contractor unless it is
agreed with the BPWS that their waste disposal system may be used.7.

piles and agree to abide by BPWS’s method statement. If I do not work in line
with this method statement then I will be removed from site.
