GUIDELINES IN THE PREPARATION OF ELECTRONIC STUDENT'S EVALUATION FORM

INSTRUCTIONS
Please do the following to maximize the use of this electronic tool:

1. CREATE a copy of the FILE using SAVE AS to retained Mastercopy and to use a new file. RENAME the file using the
conventional way, the STRAND plus the SECTION plus the YEAR (ex. STEM-EINSTEIN-2018 or other names which are
unique for easy Identification. SELECT location where to save the file then CLICK Save.

2. REPLACE entries such as Name of Division, Name of School, Address of School, and other informations.

3. SELECT from the drop down list the subjects offered for each semester arranged chronologically (according to the
class periods when the subjects were offered) starting from FIRST SEMESTER of Grade 11 up to SECOND Semester
of Grade 12. This is to create a copy which will be used for REPLICATION according to the number of SHS Learners in
your section.
PLEASE TAKE NOTE, subjects in the TVL with longer number of HOURS requirements which are taken in different
semesters (ex. Subjects with 640 hours allotment in which schedule are broken down to different semesters) shall
be entered under the corresponding SEMESTER when the total number of hours are met. For example,
Horticulture (640 hrs), is distributed in 4 semesters with 160 hours per semester, and is expected to be completed
in the second semester of Grade 12, shall be entered only once under GRADE 12, SECOND Semester.

POPULATE from the SUMMARY OF THE SUBJECTS PASSED the SPECIALIZED Subjects which will serve as checklist
for all subjects taken using the drop down list
4. When all the NECESSARY DATA on the required field are entered, REPLICATE the SHEET equal to the number of
learners.

5. Upon REPLICATION, when a prompt is diplayed on screen just CLICK Yes repeatedly until a SHEET is created.

6. RENAME each created sheet using the SURNAME and First letter of the FIRST NAME of the learners (Ex. AGACITA
R.)
7. ACCOMPLISH each evaluation form by entering GRADES corresponding to the subjects under CS Ave Column.
REMARKS Column will be automatically generated (either PASSED if Final Grades are 75 and above and FAILED if
below 75. Also the corresponding NUMBER OF HOURS for each subject will be generated if the REMARKS is PASSED
and remained blank if FAILED. TOTAL NUMBER OF HOURS in a Semester is also automatically generated.

8. SUMMARY OF SUBJECTS PASSED is also generated automatically. Subjects with FAILING MARKS will be marked
"X" while PASSING MARKS with a "/". The learner is a CANDIDATE for SHS Graduation when all subjects are "/".
