1. What service areas is your department/agency responsible for? Which of these responsibilities are
considered essential/core services? Describe your department/agency structure and its sub-
divisions. If necessary, provide an organizational chart.

2. What are the primary preparedness/mitigation/response/recovery roles (if any) of your


department/agency? Who is responsible for coordinating/leading these activities and what are
their contact details?

3. Who is your department/agency accountable to? Are there any changes to your lines of authority
or accountability during times of major emergency or disaster?

4. In what types of emergencies or disasters would you envision your department/agency being the
lead agency at the site or impacted facility? Would you see your department/agency planning a
role in the EOC, if one is activated?

5. Should your department/agency be represented in the EOC? If so, in which EOC function? What
would be your primary responsibilities? Which staff have been identified to work in the EOC?

7. What are your department’s/agency’s procedures (initial priorities) if a major emergency or


disaster occurs? What further priorities/actions would your department/agency take following
such an incident?

8. What additional procedures or guidelines does your department/agency have that are related to
emergency management?

9. How will you communicate with department/agency staff during and immediately after an
emergency (e.g., cell phones, portable radios, staff call-back policy, staff emergency procedures,
etc.)?

10. How would your staff be notified of an EOC activation? What are the activation procedures and
required authority?

11. Where do employees assemble/report for further instructions following a major emergency or
disaster? Has this been communicated to employees?

12. What is the process and authority for activating off-duty personnel? How often is the staff contact
list updated? Who is responsible for doing this?

13. What other departments or external agencies will you need to communicate with during an
emergency event? What methods of communication will be used? For what reason would you
communicate with them?

14. How will your personnel communicate with the EOC, if one is activated (e.g., via dispatch,
telephone/radio, etc.)?

15. What type of department/agency information, if any, will be important to share with the public or
other stakeholders after a major emergency? How will this be handled?

adequately respond to most types of emergencies? If not, from where would you obtain additional
resources? How would this support be coordinated?

related to emergency management? What do these agreements involve?

18. What information, equipment, reference documents, forms, tools, maps, etc. would your
department/agency need to fulfill its role in the EOC? Where are these kept? Who is responsible
for maintaining them?

19. What emergency training has your department/agency staff received? Do personnel know their
emergency roles and responsibilities?

20. What emergency response training would be beneficial to your department/agency?
