Congratulations! The College of Nursing has nominated you for the scholarship listed below. All awards take into
consideration your prior academic success and potential. The College of Nursing has made this recommendation
affirming our confidence in your abilities and in the hope that once established in your professional practice
following graduation, you will assist future nursing students through annual contributions.

Marshall Foundation for Arizona Women in Nursing

This scholarship award is provided by one of the College of Nursing's generous donors, and requires a thank you
note, which will be provided to the donor. This thank you note should be in the form of an email, with the subject
line noted as "To the Generous Donors of Marshall Foundation for Arizona Women in Nursing", and emailed
to jdmolina@email.arizona.edu<mailto:jdmolina@email.arizona.edu>. The thank you note must be received by
August 27th, or the scholarship will be revoked and given to another qualified applicant. In the note, identify your
program of study, your year of study and write a brief explanation of how the funding will affect your schooling,
and please include a recent photo that can be used to personalize your thank you to the donor.

By accepting this scholarship, you agree to the following:

§ I understand and agree to comply with the College of Nursing scholarship guidelines and I will attend any
required donor functions, which includes the College of Nursing Annual Donor Appreciation Tea, scheduled for late
April or early May 2019 (not finalized yet). If the donors have indicated they plan to attend, you will be required to
attend as well. You will be notified if your attendance is required. Please note this will be a requirement of
accepting the scholarship. Please make a note of this date now so that you can plan accordingly. Exceptions to this
attendance requirement are for students residing outside of Arizona; alternate arrangements for a personal thank
you might be requested.

§ I am aware that if offered any additional grants, loans or scholarships from the College of Nursing this award may
be withdrawn and the larger award will be given in its place. Notification will be provided if this is the case.

§ I give the College of Nursing permission to release the information submitted on my scholarship application to
the donor and to use my image for publicity purposes associated with the scholarship.
