Planning Unit such as LRN Approval, Enrolment with Gap Correction ol Grade Level'
Enrolment of lneligible/Erroneously Tagged and l\rastedist of School Head to be used
effective November 19, 2018 in which old lemplates will no longer be used nor accepted

4. Download lhe template which is suited to the Data Correclion Type for a specific learner with
pending approval. Ex. Request intended ior Erroneously Tagged leamer should not be supported with
the Template for Enrolmenl with Gap Do not edit the word 'Erroneously Tagged Template' and

replace it with the word Enrolment with Gap

5. Modifying the forms such as placing the School Name and Dislrict in the header or Crealing new
templates ihat are similar to the one provided by the Division is prohibited

6. Print the templste

7. Fill out ALL the blank boxes. Only the Efenson Name must be left blank if lh€ concemed leamer
doesn't have one. signature of concerned class Adviser and school Head should be filled out with
original slgnatu.e of boih personnel or in lheir absences, their alternateJolCs.

I The daia lo be filled out in the template should be tallied with the one encoded in lhe Learner
lnformation System and the supporting documenls. Hence, if there are discr€pandes betrrveen lhe
data on LIS and supporting documenls, justification/explanation should b6 writlen down at the
REMARKS coiumn.
