PHILIPPINES INCORPORATED, a corporation formed and organized under the laws of the
Philippines with office address at 522-Unit 504 t 508-P Five E-com Center Bldg., Pacific
Drive Extension, Block 18 Mall of Asia Complex, Pasay City, after being duly sworn in
accordance with law, depose and state, that:

1. I Am authorized to to process the retirement of business permit in your

good office which includes the authority to retire or close the business
operations, settling and submitting all pertinent documents relevant to its
retirement and do the processing of all items necessary for this closure and
authorized to make, execute, and sign any and all papers, writings or
documents, undertakings required or necessary in the processing of any or
all of the above transactions/activities

2. On July 30, 2019, the Corporation has TRANSFERRED its business location
and operation in Paranaque City from Pasay City for business reasons;

3. As such, I am executing this Affidavit to attest to the truth of the foregoing

and for whatever legal purposes that this may serve.
