AFFIDAVIT OF LATE FILING OF OLD AGE PENSION

I, ALIABABAY USMAN-HADJI GUIALAL, of legal age, widow, Filipino and

resident of Purok Kabuntalan, Tukananes, Poblacion 7, Cotabato City after being
sworn to in accordance with law, hereby depose and state:

1. That I am the same person who is the surviving wife (second wife) of the
late BALASAN BAYAK HADJI GUILAL, a WWII Veteran with rank and serial
number of PFC, #092195 INF;

2. That as such I am the beneficiary of the Old Age Pension;

3. That I was informed that there is a benefits-pension from the PVAO, so I

requested my relative to follow-up the same;

4. That I have no knowledge as to how such claim or benefits be processed

and released because I am only a farmer and I am not literate, hence this
application for old age pension was filed late;

5. That I execute this affidavit, to attest to the veracity of the foregoing facts
and circumstances for all legal purposes it may serve;

I, ALIABABY USMAN-HADJI GUILAL, of legal age, widow, Filipino and resident

of Purok Kabuntalan, Tukananes, Publacion 7, Cotabato City after being sworn to
in accordance with law, hereby depose and state:

1. That I am the same person who is the surviving wife (second wife) of the
late BALASAN BAYAK HADJI GUIALAL, a WWII Veteran with rank and serial
number of PFC, #092195 INF;

2. That as such I am the beneficiary of the Old Age Pension;

3. That however, the name of my husband was entered in the Military Service
Record as “BALASAN GUIALAR” whereas the same was entered in the
other records as “BALASAN BAYAK HADJI GUIALAL”

4. That his middle name was omitted and the word HADJI was instead entered
as his middle name, whereas the word HADJI is only a religious title
referring to “HADJI GUIALAL” and the correct middle name is “BAYAK”S

5. That the name “BALASAN GUIALAR” and “BALASAN BAYAK HADJI

GUIALAL” refer to one and identical person of BALASAN BAYAK HADJI
GUIALAL and the discrepancies were due to innocent mistake;

6. That I execute this affidavit to attest to the veracity of the foregoing facts
and circumstances for all legal purposes it may serve;
