OU-TULSA SCHOLARSHIP APPLICATION

The OU Club of Tulsa annually awards two scholarships to students attending The University of
Oklahoma-Tulsa. Each of the scholarships is $3,000.00 ($1,500.00 in cash and $1,500.00 in tuition
waiver), paid one-half in the fall semester and one-half in the spring semester. Please note that the tuition
waiver and total award amount is subject to University funding.

THE FOLLOWING CRITERIA ARE USED IN SELECTING SCHOLARSHIP RECIPIENTS:

 Applicant must presently be attending OU-Tulsa and planning to enroll in at least nine credit hours
in each semester of the next academic year.
 Applicant demonstrates achievement and leadership in college and community activities
 Applicant demonstrates financial need
 Applicant has not already been awarded a tuition waiver of more than $2,500.

Instructions: Complete all parts of this form. Mail seven (7) applications (1 original and 6 copies) along
with seven (7)) completed and signed copies of your complete undergraduate and graduate college
transcripts through the last semester completed to:
