WHEREAS, the End-user Unit submitted to the Office of the Brgy Kgd. Chairman, Committee
on Appropriation the Purchase Request for the _____________________________.

WHEREAS, the Office of the Brgy Kgd Chairman, Committee on Appropriation after verification
of the availability of appropriation and numbering of Purchase Request forwarded to the
Barangay Treasurer the said request for 3 canvassing to three (3) bonafide supplier and
prepared the abstract of canvass to determine the Lowest Calculated Responsive Bid;

WHEREAS, on the basis of the latter the BAC caused for the review and evaluation of the
proposed quotation and basic eligibility requirement of the interested suppliers;

WHEREAS, upon careful examination of their price quotation and basic eligibility requirement,

WHEREFORE, the BAC RESOLVE, AS IT IS HEREBY RESOLVED, to recommend to the
