In the US Visa Application Proforma, what should I fill in the GCI column?
GCI stands for General Company Induction and is applicable only for new entrants. Fill the dates of GCI
that has been given to you by the Recruitment team.

Should I write my name as mentioned in the passport?

Yes. For eg. If your name is Anthony Xavier Fernandes, your passport will have Fernandes typed under
the Surname column and Anthony Xavier under ‘Given Name(s)’

My Passport address is different from the address where I stay. Which address should I write?
Please mention the address where you are staying presently. If your address is temporary, then you
may mention your permanent address.

Why should I provide the Home/Mobile Number or which number should I mention?
On occasion, the consulate may call you up to verify any detail. Hence it is necessary for you to
provide all the numbers where you can be reached.

What is the meaning of Spouse?

Spouse means your wife/husband depending whether you are male or female. Please mention the
details of your spouse even if it is not mentioned in your passport. If you are a divorcee, please
provide details of your ex-spouse.

What should I mention in the Rank/Designation column?

If you a returning crew, please mention the rank that you have signed off. If you are a new entrant,
mention your last designation in the organization that you worked. Kindly avoid any short forms.

What should I write in the Salary column?

If you are a returning crew, mention your salary in Dollars/Pounds and circle the appropriate
currency. If you are a new entrant, convert your salary into dollars/pounds by visiting www.xe.com.

What should I mention in the "Explain briefly your job on board" or "Explain briefly what job you
are/were doing"?
Briefly explain the nature of job that you do on board or in the last organization that you worked. For
eg: If you are a Waiter, mention “Taking food orders from guests in a restaurant and serving them”

What should I mention under countries visited?

Mention the names of the countries that you have visited from the Immigration stamps in your
passport. Do not write names of cities instead write only the name of countries.

I have visited USA but do not remember the dates. What should I do?
Please leave the space blank if you do not remember the dates of your US visit.

What should I mention in the Work experience column?

If you are a new entrant, mention a maximum of 2 organizations that you worked including the
present and the previous one. Returning crew, please mention the names of two organisations where
you worked prior to joining the ship
