INVESTIGATION REPORT - PRESS

Decedent: Tashonna L. Ward 20-00048

SYNOPSIS
A 25 year old female with limited medical history drove to an Urgent care and collapsed near her vehicle after having been seen at
Froedtert Hospital. She was transported back to Froedtert Hospital where she was pronounced deceased. TK

DESCRIPTION OF INCIDENT
On 1/2/2020 at 2255 hours, Froedtert Hospital ER RN Timmeka Swododa reported the death of Tashonna L. Ward (MRN:
10901159), B/F, DOB: 3/18/1994, residing at 3933 N. 76th St. #2 Milwaukee, Wisconsin 53222. RN Swododa reported on
1/2/2020 at 1658 hours, the decedent presented to the Froedtert Emergency Room with her sister. She complained of having chest
pain. Multiple tests were performed including an EKG (electrocardiogram) which was normal, and a chest x-ray which showed the
decedent had cardiomegaly. Lab work was also performed which was all normal besides her bicarbonate level was 21. At 1927
hours, the decedent left the ER as she felt she was waiting too long and decided to go to an Urgent Care.

At 2039 hours, Froedtert staff called the decedent's cell phone to check on her and her sister answered. Her sister told staff the
decedent had collapsed by their vehicle after arriving to Urgent Care. She stated Tashonna was currently in an ambulance and was
being taken back to the Emergency Room. The decedent arrived back to the ER at 2107 hours. She was unresponsive upon arrival
and no additional tests were able to be performed. She continued to decline and death was pronounced at 2125 hours by Dr.
Colleen Crow. No trauma was noted to the decedent.

On 1/2/2020, I spoke with Gary with WI Diagnostics Lab and placed the antemortem specimens on hold for this office. TK

FAMILY CONTACT
On Jan 02, 2020 at 2315 hours, Taylor Kroes, D-ABMDI of the Medical Examiner's Office spoke to the decedent's mother, Yolanda
Ward, and obtained the following information:
I attempted to call Yolanda, but her phone went to voicemail. I could not leave a voice message as the mailbox was full. TK

On Jan 03, 2020 at 0010 hours, Taylor Kroes, D-ABMDI of the Medical Examiner's Office spoke to the decedent's mother, Yolanda
Ward, and obtained the following information:
I offered Yolanda my condolences and explained the role of the Medical Examiner's Office. Yolanda stated she last spoke with the
decedent over a text message around 1600-1630 hours. The decedent complained of chest pain at that time and told Yolanda she
was going to go to the hospital. Tashonna had sprained her ankle last month, but Yolanda did not know of any other recent injuries
or accidents.

I was informed the decedent had a miscarriage in March of 2019. According to Yolanda, the decedent was due in March, but the
baby was breach and the umbilical cord was wrapped around their neck. The baby died at West Allis Medical Center due to these
complications. The decedent did develop cardiomegaly during her pregnancy.

Yolanda informed me she would like an autopsy performed to determine the cause and manner of death. I voiced my understanding.
She stated she would work on choosing a funeral home in the near future. TK

SOCIAL HISTORY
On Jan 03, 2020 at 0010 hours, Taylor Kroes, D-ABMDI of the Medical Examiner's Office spoke to the decedent's mother, Yolanda
Ward, and obtained the following information:
Yolanda stated the decedent did not smoke cigarettes. She drank alcohol occasionally and would use marijuana. TK
