STUDENT AGREEMENT AND LIABILITY WAIVER

KNOW ALL MEN BY THESE PRESENTS:

This Student Agreement and Liability Waiver made and executed this
November 2019 at Ark Technological Institute Education System Inc. by Jessica
Hirang ,Filipino , and resident of Brgy. Ransohan Lucena City and her/his
guardian,Mary Grace Hirang, of legal age and Brgy. Ransohan, Lucena City
Witnesseth:
THAT above-named student will undergo an work immersion program with the
Quezon Premier Hotel for a maximum of 240 hours as part of his/her academic
requirements.
THAT the Institution is granting permission for the student to undergo said
practical training at the above-mentioned Quezon Premier Hotel subject to the
following terms and conditions:

1. During the period of his/her work immersion, the Institution shall not
provide financial assistance to the student;
2. The Student shall abide by all rules and regulations imposed by the work
immersion coordinator and the Quezon Premier Hotel and shall exercise
care and diligence in the performance of his/her tasks during the work
immersion;
3. The Institution shall not in any way be held liable for injuries and/or
accidents the student may suffer during the duration of his/her work
immersion;
4. The Institution shall be held free from any liability for damage or injury to
third parties which may be due to the intentional or negligent acts of the
Student;
5. This agreement and liability waiver shall be in effect from November 2019
until he/she finishes his/her work immersion hours.

That the above-mentioned student shall perform the following tasks in our institution/

ASSISTING CUSTOMER, SERVING , CLEANING THE SPECIFIC AREAS
, HELPING CHEFS AND CREWS ,PREPARING TABLES AND CHAIR SET-
UP , SKIRTING , COUNTING KITCHEN TOOLS,WASHING THE DISHES,
BAKING , COOKING, HOUSEKEEPING,
2. That the above-mentioned ARK student is not allowed to work on overtime or during
holidays, Saturdays and Sundays.
3. That our thru Quezon Premier Hotel the undersigned department head takes
responsibility of the above-mentioned ARK student under the “principle of loco parentis”.
4. That our Quezon Premier Hotel through the undersigned department head takes
responsibility of the above-mentioned ARK student while he/she is on scheduled duty
or performing official functions/work outside the confines of our office under the
“principle of loco parentis”.
5. That our Quezon Premier Hotel shall not in any way be held liable for injuries and/or
accidents that the student may suffer during the duration of his/ her work due to the
latter’s intentional or negligent acts;
6. That our Quezon Premier Hotel through its department head, student shall comply with
school evaluation requirements for the student volunteer.
