2017 National External Quality Assessment Scheme for Clinical Chemistry

(NEQAS-CC)
GENERAL INFORMATION

1. Submit the following in Hard Copy to LCP-NRL (DEADLINE: JUNE 15, 2017)

1.1 Fully accomplished Registration Form 302

1.2 Order of Payment Form 304 and 305
1.3 2017 Clinical Laboratory License to Operate (LTO)
1.4 Machine validated Transaction (Deposit) Slip - if payment is made via Bank to Bank Transaction

2. REMINDERS: DON’T BEAT THE DEADLINE

2.2 Incomplete requirements shall NOT be processed.

2.3 Registration Forms sent via email or fax are NOT ACCEPTABLE.

3. Participation fee is P 8, 000.00 (Department Memorandum No. 2009-0086-B)

3.1 Payment can be made via Bank to bank transaction or at LCP-Cashier.

3.2 Payment may be paid in cash or check (payable to LUNG CENTER OF THE
PHILIPPINES). Personal check is NOT acceptable.
3.3 Participation Fee for 2017 NEQAS is NON-REFUNDABLE, NON-TRANSFERABLE and
CANNOT BE USED AS ADVANCED PAYMENT FOR NEXT CYCLE.

4. Laboratory testing is limited only to the 10 Analytes (choice of 1 or more from among the following):

Glucose Total Protein Triglyceride (for the 2017 NEQAS Cycle)

Blood Urea Nitrogen Albumin
Creatinine Sodium
Uric Acid Potassium
Cholesterol Chloride

4.1 Only 1 Method and 1 Equipment shall be registered only per analyte.
4.2 Additional Method, additional Equipment enrolled per analyte will be considered as a
new transaction / participation and shall be charged another participation fee of P 8,000.00

5. Certificate of Participation (COP) issued by LCP-NRL-CC will ONLY be given to laboratory

participants who have completed the year cycle and have PAID all unsettled accounts.
