APPLICATION FOR PROBATION

ACCUSED, xxx, , unto this Honorable Court, hereby applies for probation
under the provisions of P.D. 968 as amended, and further avers as follows:

1. That accused was charged for the crime of Direct Assault upon an Agent
of a Person in Authority;

2. That on 27th of April 2018, she was convicted by the Honorable Court for
the lesser offense and was accordingly sentenced to suffer the penalty of
fifteen (15) days of imprisonment;

3. That this application is filed within the prescribed period by law;

4. That accused is out on bail and can be summoned in her given address;

5. That she is not among those who are disqualified offenders as

enumerated under Section 9 of P.D. 968 as amended;

6. That she undertakes to comply with such terms and conditions as this
Honorable Court may impose if this Application for Probation will be
granted.

WHEREFORE, premises considered, it is most respectfully prayed before

the Honorable Court that herein Application for Probation be GRANTED after
due consideration;
