Epidemiology/Biostatistics Short Course-Application form
This form is to be completed by applicants for the short course in Epidemiology and Biostatistics. All applications
should be sent via email to smusoke@uvri.go.ug and copy to jnanseko@uvri.go.ug by the 13th December 2019

Applicants should seek endorsement of their application through their respective Nodes. Normally training
opportunities are distributed to each of the five nodes of EACCR-2 (i.e. TB Node, HIV node, Malaria node, Neglected
Infectious diseases node and the Training node) in the six countries. Successful applicants will be notified via their
respective Node coordinators. Selection will be based on merits such as evidence of career path, usefulness of the
course to the candidate’s career and the likelihood of applying the learned knowledge/skills.
