AFFIDAVIT OF COMPLAINT

The UNDERSIGNED, anonymous hacker accuses cyber crime, of hacking and

locking almost 5,726 files by CryptoWall, the anonymous account also asked for a
ransom for files to be retrieved, committed as follows, to wit:

That on or about month of December, at about 7:00 in the evening, Sampaloc,

Manila, Philippines, the said accused did then and there willfully, unlawfully, feloniously,
and by means of hacking, committed by anonymous hacker, upon the undersigned
directly by overt acts to wit: (state the details how the crime was committed).
The undersigned executed this affidavit to attest the truthfulness of the foregoing
facts and to support the filing of Criminal Cases against anonymous hacker for
violations of Section 33, Republic Act No. 8792 or the E-Commerce Law of the
Philippines.

The UNDERSIGNED, an impersonator accuses identity theft, of obtaining her

credit card and using her name, address and social security number, committed as
follows, to wit:

That on or about month of January, at about 7:00. in the morning Tondo, Manila,
Philippines, the said accused did then and there willfully, unlawfully, feloniously, and by
means of identity theft, committed by an impersonator, upon the undersigned directly by
overt acts to wit: (state the details how the crime was committed).
The undersigned executed this affidavit to attest the truthfulness of the foregoing
facts and to support the filing of Criminal Cases against A local company of shoes for
violations of Section 32, Republic Act No. 8792 or the E-Commerce Law of the
Philippines and the Section 4, Republic Act No. 8484 or the Access Devices Regulation
Act of 1998 of the Philippines.

Nike, of copying their shoe line knock-off and called it “Mike”, committed as follows, to
wit:

That on or about month of January, at about 8:00 in the evening Manila,

Philippines, the said accused did then and there willfully, unlawfully, feloniously, and by
means of copyrighted, committed by brand Mike, upon the undersigned directly by overt
acts to wit: (state the details how the crime was committed).
The undersigned executed this affidavit to attest the truthfulness of the foregoing
facts and to support the filing of Criminal Cases against A local company of shoes for
violations of Section 173.2, Republic Act No. 8293 or the Intellectual Property Code of
the Philippines.

posted on social media without consent, committed as follows, to wit:

That on or about month of January, at about 9:00 in the morning, Sta. Mesa,
Manila, Philippines, the said accused did then and there willfully, unlawfully, feloniously,
and by means of cyber theft, committed by an anonymous account, upon the
undersigned directly by overt acts to wit: (state the details how the crime was
committed).
The undersigned executed this affidavit to attest the truthfulness of the foregoing
facts and to support the filing of Criminal Cases against anonymous account for
violations of Section 177 Part IV of R.A. 8293, or the Intellectual Property Code of
the Philippines.
