This survey is in reference to Cristina Nicole Bishop, age 25, and her time & punctuality habits.
The people receiving this are classmates, coworkers, colleagues, teachers, employers, friends, or
family of the individual. Cristina is conducting a Self-Improvement Project as part of an Ethics
course at Madison Area Technical College this fall (2010), and this survey is the foundation of the
research for her project. Please read through and fill out this survey to the best of your ability.

Message from Cristina: It is my goal to establish the definition of “ethical use of time” and learn to
practice the concept effectively. To accomplish this I need your help. I hereby swear to hold no ill
will, attitude, or debate towards or with any individual who honestly completes this survey. This is
your chance, to be 100% honest, and lay it all out there. My lateness, and bad concept of time,
affects each and every one of the people in my life, and I’m ready to change that. Thank you, for
