Citation Text Based on observations and interviews, the facility faled to provide housekeeping services necessary to maintain a sanitary, clean and comfortable environment thraughout the facity Specifically, the facility failed to ensure housekeeping staff was cleaning the fac effectively using crre Standards of practice. Findings include: Resident interviews A group interview was dane on 2/21/19 at 9:56 a.m. with Residents #7, #8, #9 ang #10. Resident #10 salé housekeeping only comes into his room twa to three times a week. Resident #8 said the housekeepers dant clean the bathrooms properly, the floor around the toilet is often dirty. Resident #9 sale if a room looks clean then housekeeping will nat go inthe room. Resident #11 was interviewed on 2/21/19 at 2:36 p.m. She sald the bathroom is usually dirty. She said the floor around the tollet bow, the vanity and the closet floor were brown in color. she said they do not scrub, they just mave a mop across the oor spreading the cit. Resident #12 was interviewed on 2/21/19 at 2:49 p.m. She said housekeeping uses mops that are dity and dusting was nat done unless you ask for It. She said she had to ask the housekeeper to wipe off her over the bed table that she eats off of. She said she would not have done iif she had not asked. Observations (On 2/21/19 at 3:00 p.tn. room #1008 was inspected. The vanity was cluttered with personal items in a bin with adult briefs, a halebrush and a toothbrush, ‘There was a smudge of dried brown matter on the front left side of the vanity. On the wals on elther side ofthe vanity and sik were dried light brownish colored spots. The binds, dresser and television was covered ina thick grey dust which let an obvious mark when wiped away with a finger. The bathroom had a blue seat riser over the toilet. Under the rser was dried orange discoloration aroun the underside of the seat. The back ofthe tollet was dirty with bits of tolet paper anc a dried brown matter. The sides and bottorn of the tollet had dark brown ould up around the base with a small brown smudge on tre front ofthe sear riser The room was inspected again with the director of environmental services (DES) at 3:35 p.m. He donned gloves and lifted the blue riser seat {0 find the orange discoloration under the riser. He confirmed the room was dusty and the tollet was not cleaned to the facility standards (On 2/21/19 at 3:45 p.m. room #908 was inspected with the DES. The bathroom ha¢ a black toilet seat, under the seat was dried orange discoloration around the bowel and underside ofthe seat. The floor infront of the toilet was stichy. The back ofthe toilet seat was dirty with bits of tollet paper and a dried brown substance. Around the bottom of the tollet had a dark brown build up around the base. Again the DES said the room was not cleaned to his standards or the expectations of the facility (On 2/22/19 at 8:08 a.m. housekeeper #1 was observed deep cleaning room #2111, She knocked on the door, put gloves on and turned the water in the sink ‘on, She wet a cloth with water then sprayed the cloth with Virex (a disinfectant) and proceeded to wipe dow all of the surfaces in the room including the ‘ver-the-bed tables, night stands, dressers and window sls, She then rinsed the cloth in the sink, She emptied the trash from the trash can. She sprayed the trash can with Vex and immediately wiped the inside of the trash can then the autside of the trash can, The housekeeper then changed her gloves and obtained a clean cloth, went into the bathroom and removed the toilet riser from over the tollet anc sprayed the inside and outside ofthe tollet with Crew {another disinfectant). she immediately wiped the inside of the tollet with a brush and then wiped the rest ofthe toilet down with the cloth, She placed the toilet riser back over the toilet and did not clean ‘At 8:22 a.m. housekeeper #1 then dry mapped the room, moving the furniture ané sweeping behind and under the furniture. She removed the call light cord from one of the beds and clipped it on the wall autlet when she moved the bed out to sweep behind it. She did not replace the cord when she pushed the Ded back into place. The house Keeper then took all ofthe personal care items sitting on the counter by the sink and placed them on the floor. Sne sprayed the counter and the sink with Prominent (cleaner but not disinfectant) and immediately wiped it of, she then took some of the hand soap from the aispenser on ‘the wall and washed the sink with this soap. She wiped the sink and counter aff with paper towels, she replaced the items off the figor ack onto the counter. She took the same cloth she wiped the sink and counter with and proceeded to wipe of the tops of picture frames hanging inthe room. The housekeeper then mopped the floor fram ane side of the raom to the other, going into the bathroom and mopping around the toilet, then coming out of the bathraom into the room to the doorway of the room. She had changed her gloves one time during the whale cleaning process. Staff interviews Housekeeper #1 was interviewed on 2/22/19 at 8:42 p.m. She said with each of the three cleaners she uses, she just sprays it on and wipes it off, She did not know the enemeat had to st on the surface fora specified dwell time, ‘The director of environmental services (DES) was interviewed on 2/21/19 at 3:29 p.m. He sad the facility employed 12 housekeepers, six of them being part time that worked from §:30 a.m. to 2:30 p.m. He said they had two types of room cleaning, a five step and a seven step. He sai the five step is what the housekeepers do every day. This included cleaning the tole, the sink, empty the trash, spot clean frequently touched surfaces and check supplies. The seven 9 was done manthly, with new admissions and eischarges, or if requested by the resigent or nursing Staff. This included thoroughly cleaning all surfaces in 1e room, moving the furniture and cleaning behind and under it and cleaning the closets along with the otter five steps dane daly “The DES was interviewed again on 2/22/219 at 9:35 a.m. He said Virex was a germicidal cisinfectant that should be used on frequently touched surfaces, the sink and the counter, He saic the Virex had a three minute dwell time before it should be wiped off ofthe surface. He said Crew was a cleaner and tlsinfectant to be used on the toilet. He sai it also had a dwell time of three minutes. He sais the Praminent was a cleaner but ot a disinfectant and should be used on surfaces that are not frequently touched such as window sils and hanging picture frames. He said the prominent dié not have a dwell time. He said they did quality control inspections on al of the deep cleaned rooms and rooms that received a complaint or concern about. He said the facility would be having retraining sessions done and would be follawing behind the housekeepers and going over their cleaning methods.
