Fill in the attained or predicted score of a standardized examination or a country specific university entrance examination if taken. Certified copy of the original transcript/document(s) should be sent to the Admissions Office directly from the issuing school/institution.

Applicants who are attending a high-school or secondary school whose primary medium of instruction is not English will need to satisfy one of the requirements listed on Admission Requirements. Fill in the information of your evaluators and send the “Credential Evaluation Form” to each of your evaluator. Please keep in mind to fill out the top section labeled “For the Applicant” before handing it to your evaluator. Please attach two essays for the two questions respectively. (Each essay should be 400-600words)
Part 16: Declaration
Applicants should check the application form to make sure that all sections are completed. The application form should be signed for verification. An application form that does not bear the signature of the applicant will not be processed.

This is optional and helps the Admissions Office to improve the Admissions process. Application Fee Payment Paper-based applicants are able to pay the application fee only after the admissions office (AO) receives the application. AO will inform you the log-in ID and password which you need for online payment with credit card. Please make sure your application will arrive at AO ahead of time so that you can complete your application fee payment fee by the deadline.
